Secure architecture to manage EHR’s in cloud using SSE and ABE

Background:With regard to the high cost of the Electronic Health Record (EHR), in recent years the use of new technologies, in particular cloud computing, has increased. The purpose of this study was to review systematically the studies conducted in the field of cloud computing. Methods:The present study was a systematic review conducted in 2017. Search was performed in the Scopus, Web of Sciences, IEEE, Pub Med and Google Scholar databases by combination keywords. From the 431 article that selected at the first, after applying the inclusion and exclusion criteria, 27 articles were selected for surveyed. Data gathering was done by a self-made check list and was analyzed by content analysis method. Results:The finding of this study showed that cloud computing is a very widespread technology. It includes domains such as cost, security and privacy, scalability, mutual performance and interoperability, implementation platform and independence of Cloud Computing, ability to search and exploration, reducing errors and improving the quality, structure, flexibility and sharing ability. It will be effective for electronic health record. Conclusion:According to the findings of the present study, higher capabilities of cloud computing are useful in implementing EHR in a variety of contexts. It also provides wide opportunities for managers, analysts and providers of health information systems. Considering the advantages and domains of cloud computing in the establishment of HER, it is recommended to use this technology.

Electronic medical records can help people prevent diseases, improve cure rates, provide a significant basis for medical institutions and pharmaceutical companies, and provide legal evidence for medical negligence and medical disputes. However, the integrity and security problems of electronic medical data still intractable. In this paper, based on the ciphertext policy attribute-based encryption system and IPFS storage environment, combined with blockchain technology, we constructed an attribute-based encryption scheme for secure storage and efficient sharing of electronic medical records in IPFS storage environment. Our scheme is based on ciphertext policy attribute encryption, which effectively controls the access of electronic medical data without affecting efficient retrieval. Meanwhile, we store the encrypted electronic medical data in the decentralized InterPlanetary File System (IPFS), which not only ensures the security of the storage platform but also solves the problem of the single point of failure. Besides, we leverage the non-tamperable and traceable nature of blockchain technology to achieve secure storage and search for medical data. The security proof shows that our scheme achieves selective security for the choose keyword attacks. Performance analysis and real data set simulation experiments shows that our scheme is efficient and feasible.

Background. The incidence of coronary artery disease (CAD), the leading cause of mortality in most developed and developing countries, is increasing. The adoption of hospital registries can improve care delivery and facilitate the management of CAD through better planning, as well as help with outcome assessment through more effective data management. Objectives. The present study is aimed at designing a hospital-based CAD registry for managing CAD data. Methods. This developmental study was conducted in three phases. Initially, sources related to CAD registries were reviewed, the results of which were published in two studies. In the next phase, the prerequisites and requisites of the software were determined through a qualitative study. In this phase, the registry dataset was determined by using a questionnaire. Finally, the developed conceptual model of the software was validated. The software was then developed based on the validated conceptual model. Results. The registry data elements were classified into 13 main categories, including identification data, medical history, and risk factors. The dataset included 171 data elements, including data related to surgical and nonsurgical procedures. The conceptual model was approved by field experts, and the software was developed accordingly. Conclusion. The steps followed in the present study for developing the CAD registry can be used as an appropriate approach for designing similar hospital-based registries. Considering the pivotal role of the registry in the management of CAD, the routine and systemic use of the registry is suggested in all healthcare centers.
