Sebaceous carcinoma of the eyelid: a clinicopathological study

Abstract: Background-Sebaceous carcinoma of the eyelid is rare. The diagnosis might be diYcult because of its ability to masquerade as other periocular lesions. Prognosis is still regarded as being poor compared with most other malignant eyelid tumours with a mortality second only to malignant melanoma. The present study retrospectively analyses clinical and histopathological findings and outcome in a series of patients with sebaceous carcinoma of the eyelid in Britain. Methods-43 patients with histologically confirmed … Show more

“…Studies have been performed to investigate such markers in tumors with sebaceous differentiation. 11,14,15,17 In our study, we examine a series of tumors that can enter the differential diagnosis with sebaceous neoplasia for expression of adipophilin. We show vesicular staining for adipophilin in the sebocytes of normal sebaceous glands, in 16 of 16 (100%) sebaceous adenomas and 23 of 25 (92%) sebaceous Table 3 Eyelid lesions: grading of adipophilin staining results carcinomas, including 9 of 11 (82%) cases of poorly differentiated sebaceous cell carcinomas.…”

“…It is particularly important to differentiate these entities because their behavior is generally less aggressive than sebaceous carcinoma. 5,11 Other lesions with clear cell histology, such as xanthomatous lesions, tumors originating in the outer root sheath cells of hair follicle, balloon cell nevi, and, sometimes, metastatic renal cell carcinoma can be difficult to distinguish in small biopsy specimens.…”

“…The majority of all recurrences appear within the first 4 years after treatment. 18 The rate of metastasis at 5 years was 16.1%. Four of our patients with SGC developed regional lymph node metastasis and received parotidectomy, radical neck dissection, and chemotherapy or radiotherapy.…”

“…According to the literature, distant metastasis affects 14-25% of the cases and involves lymph node or hematogenous spread into liver, lungs, brain, and bones. 18 SGC is traditionally considered among the most lethal of all tumours of the ocular adnexa. Mortality from SGC has been estimated to be from 6 to 30% in a previous study.…”

“…[5][6][7] Pagetoid invasion of the conjunctiva can be detected by conjunctival map biopsies, 8 and may be an indication for exenteration, although a recent paper has reported the effective use of topical Mitomycin-C in such patients. 9 This patient had a very small tumour, but pagetoid spread meant that 75% of her upper eyelid was excised to clear this.…”
