Background. The seasonal variability of blood pressure (BP) and cardiovascular morbidity and mortality motivates researchers to search for the new approaches of the antihypertensive treatment (AHT) efficacy assessment, including the analysis of interrelation between clinic and ambulatory BP, i.e. BP phenotypes. Aim. To evaluate the prevalence of BP phenotypes in treated hypertensive outpatients both in winter and summer. Material and methods. Hypertensive patients under regular AHT with available data of clinic BP and 24-hour BP monitoring both in winter and summer seasons (n=477) were selected from a outpatient database with clinic BP <160/100 mm Hg (n=1762). The BP threshold values from current guidelines were used for the BP phenotypes definition. The "controlled hypertension" phenotype was detected when clinic and ambulatory BP levels were less than threshold values, the "uncontrolled hypertension" -when both levels exceeded the thresholds, the "white coat hypertension (WCH) in treated patients" -when clinic BP was only increased, the "masked uncontrolled hypertension" phenotype was ascertained if ambulatory BP was only elevated. Results. The study included 232 patients examined in Ivanovo (a relatively cold region; mean age 53.5±9.3 years; 28% men; clinic BP 121.7±7.9 mm Hg; mean duration of hypertension 4.6±6.3 years) and 245 patients from Saratov (a relatively hot region; mean age 58.3±10.6 years; 56.3% men; clinic BP 127.5±14.9 mmHg; mean duration of hypertension 9.2±9.2 years; р<0.0001). In the Ivanovo cohort, ratio of BP phenotypes in summer and winter did not differ; the "masked uncontrolled hypertension" phenotype prevailed (62.1% and 63.8% in winter and summer, respectively); AHT was effective in 27.6% and 29.7% of the patients in winter and summer, respectively. The prevalence of other phenotypes was low: uncontrolled hypertension -9.5% in winter and 6.0% in summer; WCH in treated patients -less than 1% in both seasons. The prevalence of BP phenotypes in the patients examined in Saratov differed in winter and summer (χ 2 =18.127, р<0.0001); the "masked uncontrolled hypertension" phenotype rate was higher in summer than in winter (50.2% and 32.2%, respectively); the rates of other phenotypes were higher in winter: uncontrolled hypertension -38.8% and 27.8%, controlled hypertension -25.3% and 20.8%, WCH -3.7% and 1.2% in winter and summer, respectively. Conclusion. We have found no seasonal changes in BP phenotypes prevalence in relatively younger hypertensive patients in a relatively cold region. The "masked uncontrolled hypertension" phenotype was predominant (>60%) in this region. In contrast, the cohort of relatively older patients with longer hypertension history examined in a relatively warmer region revealed significantly higher rate of masked uncontrolled hypertension in summer (50.2%) while prevalence of all other phenotypes -in winter.

Цель. Оценить влияние температуры, влажности, загрязнения воздуха на смертность населения г. Москвы в 2007-2014гг, определить возможность прогнозирования последней с помощью метода математического моделиро-вания. Материал и методы. За каждые сутки 2007-2014гг проанализированы дан-ные о смертности населения г. Москвы, температуре, влажности и загрязне-нии (рМ 10 ) воздуха. Число случаев смерти составило 956436 (48,1% мужчин и 51,9% женщин), из них от БСК -536625. использовался метод многофак-торного регрессионного анализа с применением обобщенной логистической регрессии, оценкой отношения рисков (Ор), 95% доверительного интервала. Применены прогностические модели, разработанные на основании нелиней-ного многофакторного анализа. Результаты. фактор "волна жары" повышал Ор смерти от всех причин и от БСК в 1,43 и 1,65 раза. Волны жары в большей степени повышали риск смерти от всех причин, от БСК у женщин (р<0,005), увеличение загрязненно-сти воздуха (рМ 10 ) -у мужчин (р<0,005). Наибольшее возрастание риска смерти при аномально высокой температуре и загрязнении воздуха выявлено у лиц возраста 70 лет и старше, дополнительно увеличивало риск смерти повышение влажности воздуха. Точность прогнозирования смертности была выше для лета 2010г, чем для такового 2014г (корреляция 90,1% и 45,1%), т. е. периода с большей выраженностью аномальных отклонений температуры и загрязнения воздуха. При применении многофакторной прогностической модели для периода 2007-2014гг с дополнительной оценкой влияния факто-ров погоды за последние 4 суток и смертности от всех причин за предыдущий месяц корреляция была выше для прогнозирования смертности на 1-3 суток (88,0%), чем на месяц (45,2%). заключение. Волны жары, возрастание температуры и загрязнения воздуха повышают риск смерти населения от всех причин, особенно от БСК, у лиц возрастной группы 70 лет и старше. Аномальное повышение температуры воздуха в большей степени увеличивает риск смерти у женщин, возрастание загрязненности воздуха -у мужчин. Прогнозирование смертности населения на 1-3 суток с использованием многофакторной модели характеризуется более высокой корреляцией, чем прогнозирование на месяц. Aim. Many patients and some doctors associate the worsening of cardiovascular diseases with meteoropathy (changes in atmospheric pressure and air temperature). Opinion about lack of weather influence on the increase in overall morbidity and the lack of related research data indicate the need to study the relationship between cardiovascular diseases and meteorological conditions.Aim. To assess the prevalence of hospitalizations with symptomatic increases in blood pressure (BP), atrial fibrillation (AF), and decompensated heart failure (HF) in a Moscow hospital depending on meteorological conditions.Material and methods. This retrospective analysis of the number of emergency hospitalizations of patients with symptomatic increases in blood pressure, AF, and decompensated HF in Moscow City Clinical Hospital № 24 was carried out in the period from October 1, 2018 to September 30, 2019. Information from the website www.weather.com was used as a source of meteorological data. The following indicators were assessed daily: daytime air temperature (t), nighttime air temperature, maximum diurnal air temperature variation, atmospheric pressure, maximum diurnal atmospheric pressure variation, amount of precipitation. To assess statistical significance, the Mann-Whitney and Kruskal-Wallis tests were used. A correlation analysis was carried out, and a prognostic model was created using multiple linear regression.Results. During the period from October 1, 2018 to September 30, 2019, 4354 patients were hospitalized at City Clinical Hospital № 24 with symptomatic BP increase and/or AF and/or decompensated HF. The highest number of hospitalizations occurred in November, December and January. The median number of admissions per day was 12 patients. When comparing the mean number of patients admitted to the hospital per day, depending on day- and nighttime air temperature, following statistically significant differences were obtained (p<0,001): with day- and nighttime air temperature ≥0о C, the number of admissions was less compared to sub-zero temperatures. Significant inverse correlations were established between the number of patients hospitalized with symptomatic BP increase, AF and HF per day, with air temperature at night (correlation coefficient r=-0,339; p<0,001), air t during the day (r=-0,316; p<0,001), temperature difference over 48 hours (r=-0,205; p<0,001); direct correlations with atmospheric pressure (r=0,106; p=0,044) and its changes (r=0,115; p=0,028) were identified. All identified relationships were of moderate strength. According to prognostic model, only nighttime air temperature turned out to be a significant indicator associated with the hospitalization rate per day. With a decrease in air temperature at night for every 7,5о C <0, one more patient is expected to be hospitalized with a symptomatic BP increase, decompensated HF, or paroxysmal AF.Conclusion. Negative air temperature in a metropolis is associated with an increase in hospitalizations for symptomatic BP increase, AF or decompensated HF. Atmospheric pressure and precipitation do not affect these conditions.
