Sealing vessels up to 7 mm in diameter solely with ultrasonic technology

Abstract: IntroductionUltrasonic energy is a mainstay in the armamentarium of surgeons, providing multifunctionality, precision, and control when dissecting and sealing vessels up to 5 mm in diameter. Historically, the inability to seal vessels in the 5–7 mm range has been perceived as an inherent limitation of ultrasonic technology. The purpose of this study was to evaluate sealing of vessels up to 7 mm in diameter with an ultrasonic device that modulates energy delivery during the sealing period.MethodsIn ex vivo benc… Show more

“…Similarly to the Ligasure, it can be used in vessels up to 5 mm. 31 When compared to other vessel sealing devices, the Harmonic achieves faster sealant. 32 In thyroidectomy, this has been shown to lead to a reduction in operating time, compared to the Ligasure.…”

“…Various new surgical devices and technologies have been introduced in the past three decades to improve the efficiency of surgical procedures—either to achieve a range of desired tissue dissection, transection, and hemostasis or to reduce operative time and postoperative complications [ 1 4 ]. One such technological advancement included the introduction of ultrasonic cutting and coagulation of soft tissues [ 5 , 6 ].…”

“…The Harmonic scalpel (HS) is a device that uses ultrasonic energy for cutting of the tissues, tissue dissection, and coagulation [ 6 ]. Numerous studies have described the advantages of ultrasonic sealing and cutting devices over conventional electrosurgery including better hemostasis with minimal thermal damage, reduced risk of nerve damage, fewer instrument changes due to the combined vessel-sealing, tissue cutting and dissecting functionality, and lower visual obstruction from mist or smoke [ 3 , 4 , 7 , 8 ].…”
