Screening of Glaucoma or Cataract Prevalence in Vitiligo Patients and Its Relationship With Periorbital Steroid Use

Abstract: The study suggests that patients who have vitiligo and use topical steroids periorbitally do not have an increased risk of glaucoma or cataracts. Future studies that have a larger sample size and use a detailed steroid use protocol are needed.

“…The coexistence of vitiligo and ocular abnormalities has been explored by several studies. 2,[50][51][52][53] Melanocytes are, in addition to the skin, abundant in the uveal tract (choroid, ciliary body, and iris) and the retinal pigment epithelium (RPE). 50 of glaucoma in a study with larger sample size (90 vitiligo patients) and examined its association with topical use of periorbital corticosteroids.…”

“…They found no significant difference in the prevalence of glaucoma or cataracts between cases and controls, even with the use of topical corticosteroids periorbitally. 53 The association between vitiligo and audiological abnormalities are highlighted in the literature. 2,28,[54][55][56] Melanocytes are also distributed in the membranous labyrinth of the inner ear with the heaviest pigmentation in scala vestibuli.…”

“…Topical corticosteroids are the mainstay of treatment for localized disease and have been shown to have a reservoir effect that lasts for around 5 days. The risk of development of glaucoma or cataracts in patients using periorbital topical corticosteroids was not found to be raised in case-control studies [ 31 ]. Topical vitamin D analogues have been used with variable outcomes in vitiligo, and the plausible mechanism of action includes immunomodulatory and melanocyte stimulating activities.…”

“…Similar lesions have been described in the literature, with reported rates of 4.5% and 6.5% in the studied vitiligo samples. 10,25 None of our patients had evidence of choroidal nevi in their fundus examination. Reports on choroidal nevi in vitiligo patients have varied, with some studies on black populations reporting 0% incidence in the studied sample 24 and others reporting up to 40% in a Jewish population.…”
