Screening for Tuberculosis in Health Care Workers: Experience in an Italian Teaching Hospital

Abstract: Health care workers (HCW) are particularly at risk of acquiring tuberculosis (TB), even in countries with low TB incidence. Therefore, TB screening in HCW is a useful prevention strategy in countries with both low and high TB incidence. Tuberculin skin test (TST) is widely used although it suffers of low specificity; on the contrary, the in vitro enzyme immunoassay tests (IGRA) show superior specificity and sensitivity but are more expensive. The present study reports the results of a three-year TB surveillanc… Show more

“…The lack of immunization with the BCG vaccine was independently associated with LTBI, which is in line with some findings from several studies on the effectiveness of the protection afforded by the BCG vaccine against TB and LTBI . However, there is still much to learn about appropriate BCG vaccine strategies because of conflicting reports on the efficacy of BCG .…”

“…18 A study from Rome, Italy witnessed low LTBI burden 16.1% in a teaching hospital. 8 Despite being a high-risk group, the exposure of healthcare workers to TB in Pakistan has not been studied earlier which is one of the advantages of the present study. Furthermore, we have highlighted the risk of occupational exposure of HCWs to tuberculosis infection.…”

“…No other population subgroup is more highly exposed to multi-drug resistant tuberculosis (MDR) than healthcare workers and thus, have sixfold risk to contact MDR tuberculosis. 8 While the significance of LTBI detection cannot be overemphasized, there is no gold standard test for its diagnosis. The quantity of Mycobacterium tuberculosis in LTBI patients is minimal.…”

“…According to this strategy, the IGRA test has a central role as a TB second step-screening, in order to reduce the number of people that should undergo more invasive tests (X-ray diagnosis and/or chemotherapy). TST followed by QFT was particularly cost-effective when the rate of LTBI in the screened population was low [23,24].…”
