Screening for Stroke in Sickle Cell Anemia: Comparison of Transcranial Doppler Imaging and Nonimaging US Techniques

Abstract: V(mean) measurements in the middle cerebral artery, distal internal carotid artery, and anterior cerebral artery did not vary substantially between nonimaging and imaging transcranial Doppler US. RI data did not yield comparable measurements.

“…These children were screened by TCD imaging (TCDI) 3,[5][6][7] since the age of 12 to 18 months and had a history of abnormal TCDI defined by TAMMV $200 cm/s and at least 1 year of follow-up with TCDI. Hemoglobin levels were measured the same day as the TCDI assessment.…”

“…Previous evaluation of TCD imaging versus TCD scanning had shown that both methods gave similar results when used according to strict, established protocols. 10 Extracranial carotid and vertebral arteries were routinely scanned during the examination. Although this did not form part of the scanning protocol in the STOP trial when non-imaging TCD was used, it has been used routinely in our center to provide additional information on the cerebral circulation.…”

“…Moreover, no significant difference was found in a recent report comparing both methods. 3 Thus, the study by Rees et al may have included patients with only marginally abnormal velocities. In addition, if these patients were transfused early, it could have influenced the occurrence of vasculopathy.…”

“…Proportions of immature CD19 ؉ CD21 ؊ B lymphocytes predict the response to extracorporeal photopheresis in patients with chronic graft-versus-host disease B lymphocytes are increasingly assigned an important role in the pathogenesis of auto-and alloimmune diseases including chronic graft-versus-host disease (cGVHD) [1][2][3][4] where abnormalities of Bcell homeostasis in patients with active cGVHD have been observed. 1,4 Extracorporeal photopheresis (ECP) achieves high response rates in steroid-refractory cGVHD.…”
