Screening for NAFLD—Current Knowledge and Challenges

Abstract: Non-alcoholic fatty liver disease (NAFLD) is the most common cause of abnormal liver function tests worldwide, with an estimated prevalence ranging between 19–46% in the general population. Of note, NAFLD is also expected to become a leading cause of end-stage liver disease in the next decades. Given the high prevalence and severity of NAFLD, especially in high-risk populations (i.e., patients with type-2 diabetes mellitus and/or obesity), there is a major interest in early detection of the disease in primary … Show more

“…To our knowledge, this is the first exploratory cross-sectional study evaluating the association between moderate-to-severe AD and NAFLD. The novel finding of our cross-sectional study was that the prevalence of NAFLD detected via ultrasonography in adult patients with moderate-to-severe AD was 24.1%, similar to that observed in patients with melanoma in situ (considered as the control group), and to that already reported in the general adult population [ 10 , 11 , 12 , 13 ]. The main reasons for which we chose patients with melanoma in situ as the control group are the following: (a) they represent a patient group with a non-inflammatory skin disease, different to psoriasis and AD; (b) based on the current knowledge, it is unlikely that melanoma in situ can significantly influence the development and progression of NAFLD.…”

“…Non-alcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver diseases worldwide, with an estimated global prevalence of around 30% in adults in the general population [ 10 , 11 , 12 , 13 ]. NAFLD is characterized by an accumulation of fat in the liver, and is strongly associated with insulin resistance and metabolic syndrome features [ 14 , 15 , 16 , 17 , 18 ].…”

“…Moreover, personalized medicine can play a significant role in managing NAFLD including metabolic syndrome [40]. Given that NAFLD affects more than a quarter of the population, it is impractical to screen everyone [41]. A recent study demonstrated that specific genetic variants, such as patatin-like phospholipase domaincontaining 3 (PNPLA3), or biomarkers, can possibly be used to predict the progression of NAFLD [42][43][44].…”

“…This lack of knowledge may lead to inconsistent screening practices. 103 104 105 106 Disease awareness and physician knowledge could be improved after the introduction of the new diagnostic criteria, which are more defined and simplified. Additionally, this highlights the need for further education and training for PCP on disease diagnosis and management.…”
