Older people go outdoors for various purposes, such as going shopping, taking a walk, visiting friends, and working in their garden or field. These activities are important for the maintenance of health and quality of life. With advancing age, there is a decline in the frequency of going outdoors. For example, in Japan more than 50% of older people aged 65-69 years go outside the house at least once a day, but among those aged 70-79 years and over 80 years, the respective percentages are less than 40% and 30%. 1 The clinico-epidemiologic relevance of the reduction in the frequency of going outdoors in community-dwelling older adults has not been well characterized. Getting outdoors requires a certain level of physical and mental Study PopulationThis was a two-year prospective study conducted in Yoita Town, a rural area in Niigata Prefecture, Japan. In 2000, the total population of this town was 7,493, and the proportion of older adults (i.e., 65 years and older) was 22.3%. A baseline survey was conducted in November 2000, in which all residents aged 65+ years (n=1,673) were invited to participate. Out of them, 1,544 (92.3%) persons responded to the survey. Among the 129 non-responders, 80 were institutionalized, 2 were relocated, 3 had died, and 44 refused to participate.A follow-up survey was conducted in October of 2002. Out of the initial 1,544 respondents, 1,283 (83.1%) participated again. Among the 261 non-participants, 57 were institutionalized, 12 persons were relocated, 81 persons had died, 88 refused to participate, and the remaining 23 persons did not participate for miscellaneous reasons.Trained personnel conducted standardized, face-to-face interviews with study subjects either at the community hall or in their homes, both at baseline and follow-up. The purpose of the study (collecting information on how to prevent disability among older people) was explained before interview, and all subjects participated voluntarily in accordance with a protocol approved by the Ethics Committee of Tokyo Metropolitan Institute of Gerontology. VariablesThe questionnaire comprised of sociodemographic, medical and physical function, cognitive, and psychosocial items. Sociodemographic variables included age, sex, education, principal occupation, and household structure. Medical and physical function variables included the basic activities of daily living (BADL), IADL, walking ability, visual and hearing ability, pain, chronic medical conditions, urinary incontinence, use of outpatient clinics, history of hospital admission, and frequency of going outdoors. Resumo -Objetivo: Tradução e adaptação da escala Cornell de depressão em demência e verificação da confiabilidade entre e intra-examinadores da versão na língua portuguesa. Método: A versão original da escala Cornell foi traduzida para o português por firma especializada em tradução de textos médicos e retrotraduzida para o inglês por outros dois tradutores independentes. As divergências de tradução foram identificadas e discutidas, chegando-se à versão que foi submetida à pré-teste para adaptação sócio-cultural. Após esta adaptação, obteve-se a versão final que foi administrada a amostra de 29 pacientes com doença de Alzheimer provável e aos seus cuidadores. Resultados: A versão final da escala mostrou-se de fá-cil aplicação e obteve boa confiabilidade intra-examinador (Kappa=0,77; p<0,001) e entre-examinadores (Kappa=0,76; p<0,001). Conclusão: A versão brasileira da escala Cornell é um instrumento que pode ser utilizado para avaliação e acompanhamento de depressão em pacientes com demência.PAlAvRAs-ChAve: depressão, escalas, adaptação, reprodutibilidade dos testes. Brazilian version of the cornell depression scale in dementiaABsTRACT -Objective: Translating and adapting the Cornell scale for depression in dementia to the Portuguese language and verifying the interrater and test-retest reliability of the translated and adapted version. Method: The Cornell scale was translated into Portuguese and back translated into english. Divergences of translation were identified and discussed, resulting in a version which was submitted to a pre-test for cross-cultural adaptation. The final version was administered to a sample of 29 patients with probable AD and to their caregivers. Results: The Cornell scale presented good interrater (Kappa=0,77; p<0,001) and test-retest reliability (Kappa=0,76; p<0,001). The final version was easy to administer and well understood by the caregivers. Conclusion: The Brazilian version of the Cornell scale is an instrument with good reliability to evaluate depression in patients with dementia. SUMMARYBackground Cognitive impairment and depressive symptoms impose a heavy burden on the care of the elderly in Japan and Korea, two of the fastest aging nations in Asia. The purpose of this study was to examine and compare factors associated with cognitive impairment and depressive symptoms among older persons in the two countries. Methods In 2002, representative samples of community-dwelling people aged 65 and older were selected among residents in Anyang, Korea and Yoita, Japan. Mini-Mental State Examination and Geriatric Depression Scale were used to assess the elderly's mental status. Sociodemographics, physical function, chronic conditions, social support, and health behaviors were examined to identify significant associations. Results The prevalence of cognitive impairment in older adults was 17.0% in Anyang and 14.6% in Yoita. The rates for depressive symptoms were 15.2% and 19.8%, respectively. Overall, functional capacity was the universal factor significantly associated with mental conditions. Self-rated health and social support were also found to be independently associated with depressive symptoms in the study subjects. Differences in the patterns of association by community, however, were notable for other characteristics. For example, in factors associated with cognitive impairment, sociodemographic factors such as age, gender, and education were significant among Koreans, whereas socio-behavioral factors such as obesity, social support and hospitalization experience were found to be significant for older Japanese residents. Conclusions Similarities in the patterns of association indicate the need for joint explorations into the role these factors play in affecting the mental health of older persons. Socioeconomic and regional differentials, however, may account for the disparity in the associations observed, suggesting the importance of developing mental health programs sensitive to the older individual's culture.
