Objective Feeding intolerance (FI) is a common presentation of necrotizing enterocolitis (NEC) and sepsis. NEC and sepsis are associated with hematological changes, but these changes alone are not reliable biomarkers for early diagnosis. This study examined whether the combination of hematological indices and FI can be used as an early diagnostic tool for NEC or sepsis. Study Design This retrospective cohort study included infants born at <1,500 g or <30 weeks who had symptoms of FI. The exclusion criteria were congenital or chromosomal disorders, thrombocytopenia or platelet transfusion before the onset of FI, and history of bowel resection. We compared the hematological indices from infants with pathologic FI (due to NEC or sepsis) to infants with benign FI. Results During the study period, 211 infants developed FI; 185 met the inclusion criteria. Infants with pathologic FI (n = 90, 37 cases with NEC and 53 with sepsis) had lower birth gestational age and weight compared with 95 infants with benign FI (n = 95). Pathologic FI was associated with lower platelet count (median 152 × 103/μL vs. 285 × 103/μL, p < 0.001) and higher immature-to-total neutrophil (I/T) ratio (median 0.23 vs. 0.04, p < 0.001) at the onset of FI. Pathologic FI was also associated with a decrease in baseline platelets compared with an increase in benign FI. For diagnosis of pathologic FI, a decrease ≥10% in platelets from baseline had a sensitivity and specificity of 0.64 and 0.73, respectively, I/T ratio ≥0.1 had a sensitivity and specificity of 0.71 and 0.78, respectively, and the combination of both parameters had a sensitivity and specificity of 0.50 and 0.97, respectively. Conclusion FI caused by NEC or sepsis was associated with a decrease in platelets from baseline, and a lower platelet level and higher I/T ratio at the onset of FI. These findings can help clinicians in the management of preterm infants with FI. Key Points

Introdução: A sepse neonatal continua sendo um desafio pela alta incidência e letalidade e pela dificuldade no diagnóstico, especialmente entre os recém-nascidos (RN) pré-termo de muito baixo peso. O diagnóstico precoce e o início do tratamento apropriado têm um papel crucial na melhoria da sobrevida destes recém-nascidos. Objetivo: Avaliar a presença de biomarcadores em sangue de cordão umbilical e periférico de recém-nascidos pré-termo de muito baixo peso que possam auxiliar na predição da sepse neonatal. Método: Foram dosados 27 biomarcadores com um kit de alta precisão (Bio-Plex Pro Human Cytokyne 27-plex Assay) em sangue do cordão umbilical e no sangue periférico no 2º, 7º, 14º e 28º dias de vida, dos RN com idade gestacional menor que 34 semanas e peso de nascimento inferior a 1.500g, sem malformações congênitas maiores, que nasceram durante um período de 8 meses. Os RN foram acompanhados e divididos em 2 grupos: grupo controle e grupo sepse de acordo com a presença de sepse. Para os marcadores com diferença estatística foi realizado a curva ROC e encontrado o melhor cutoff que poderia ajudar a predizer a sepse e calculado a sensibilidade, especificidade, acurácia, valor preditivo positivo e negativo. Resultados: Durante o período estudado nasceram 48 RN elegíveis. Na análise do sangue do cordão umbilical foram excluídos 9 RN por não ter sido possível a coleta do sangue no momento do nascimento, e divididos em 2 grupos: grupo controle (n=12) e grupo sepse (n=27), sendo 15 (56%) sepse clínica e 12 (44%) sepse comprovada, com média da idade no dia da sepse de 7 dias. Os RN do grupo sepse apresentaram menores concentrações de MCP-1 do que os RN do grupo controle. Os RN com valores de MCP-1 menores que 130,2 pg/mL no sangue do cordão umbilical apresentaram chance 9 vezes maior de sepse no período neonatal com sensibilidade de 81,2% e especificidade de 66,6%. Já no sangue periférico de 2 dias de vida houve um comportamento inverso. Os RN com valores de MCP-1 maiores que 111,3 pg/mL apresentaram chance 7,2 vezes maior de sepse no período neonatal com sensibilidade de 70,8% e especificidade de 75%. Na análise do sangue periférico do RN, foram excluídos os RN com sepse precoce (n=5), e os que faleceram antes do 7º dia de vida (n=5), e após divididos em 2 grupos: grupo controle (n=14) e grupo sepse tardia (n=24), sendo 14 (58,3%) sepse clínica e 10 (41,7%) sepse comprovada, com média da idade no dia da sepse de 11,1 dias. Foram então escolhidos para análise as amostras de sangue do 2º e 7º dia de vida do RN, antes do diagnóstico de sepse. No 2º dia de vida, os RN do grupo sepse tardia apresentaram maiores concentrações de IL1-ra, IL-6, IP-10, e MCP-1 e menores concentrações de FGF basic, IFN-γ, IL-4, IL-5, IL-7, IL-9, IL-10, IL-12p70, IL-13, IL-17A, PDGF-BB e TNF-α. Imagine for a moment you are counseling a patient with a suspicious mass and symptoms that are concerning but nonspecific. Although there are several possible diagnoses, one hangs unspoken between you and the patient-a diagnosis that carries significant morbidity and mortality. You propose an immediate biopsy. After all, a tissue diagnosis is the criterion standard, and this matter is urgent. The patient agrees, and as the patient begins to stand, you raise a hand and mention another test you would like to order. The patient is curious. "What are the benefits of this test?" You explain that the test involves sending a portion of the biopsied tissue to a separate part of the laboratory, where the pathology department will look for other markers of disease. Of course, the patient nods; that makes sense. "If this extra test result is negative, I will be okay, right?" "No," you answer, "a negative test result does not mean the biopsy result will be negative." The patient frowns. "Well, is it a bad sign if the test result is positive?" "Not necessarily," you explain. "Many things can make the other test results abnormal, so we'll still have to wait on the biopsy results. The biopsy is the key." The patient sits down again, looking confused. "If the biopsy is so important, why are we wasting tissue on this other test?" "Well," you glance helplessly toward the window then look back to your frowning patient. "It's just what we've always done in these cases."This exchange is hypothetical, but in the case of Creactive protein (CRP) for neonatal sepsis, it is all too real. The nonspecific signs of late-onset infection in infants, particularly those born preterm, combined with the high risk of morbidity and mortality have underscored the need for accurate diagnosis of neonatal sepsis. 1 Culture of blood and other sterile sites is the criterion standard for neonatal sepsis. However, myriad ancillary laboratory tests have been suggested as potential biomarkers for neonatal sepsis; these include CRP, complete blood counts with differential, procalcitonin, a variety of interleukins, and presepsin. 2 The value of those tests in the clinical management of suspected sepsis is questionable. In this issue of JAMA Pediatrics, Brown et al 3 report their systematic review and meta-analysis of the sensitivity, specificity, and test accuracy of CRP for late-onset neonatal sepsis. They analyzed 22 studies including 2255 infants, the majority being 32 weeks or less gestational age or 1500 g or less birth weight. The median specificity of CRP was 0.74 and median sensitivity was 0.62. Assuming a cohort of 1000 preterm infants with a 20% prevalence rate of late-onset sepsis, this means that 76 cases of infection would be missed, and 208 infants would be incorrectly diagnosed as having sepsis-more than the number of infants with sepsis (200).
