Screening acute cholangitis patients for sepsis

Abstract: Background Acute cholangitis (AC) complicated by septic shock is associated with 40% mortality. The best screening method for diagnosing sepsis in patients with AC is unknown. In this study, we aimed to compare the discriminative powers of systemic inflammatory response syndrome criteria (SIRS test) and the 2018 Tokyo Guidelines for moderate cholangitis (TG18 test) in screening AC patients for sepsis and to estimate their predictive abilities. Methods This was a retrospective diagnostic accuracy study in which… Show more

“…As a result, many hospitals have adopted new the Sepsis-3 criteria, which may affect the diagnosis and management of AC patients [ 9 ]. At the same time, some researchers have found that TG18 is less than 70% sensitive and specific in AC patients with severe sepsis [ 10 ]. So, there is an urgent need for new clinical evidence to improve the rate of early diagnosis of patients with critical acute cholangitis.…”

“…This study shows that infection is one of the causes of sepsis [ 3 ]. CAP with sepsis is characterized by inflammatory syndrome in the early stages, which is similar to the symptoms of sepsis caused by other types of disease, such as peritonitis [ 4 ], cholangitis [ 5 ], and acute bacterial meningitis [ 6 ]. Subsequently, the body produces a series of inflammatory responses to infection, in which the immune function and the coagulation system of the body undergo complex changes [ 7 ].…”
