Rosai Dorfman disease diagnosed by fine‐needle aspiration cytology in a young man with HIV infection

Abstract: Key Clinical MessageRDD (Rosai Dorfman disease) is a rare and benign histiocytic proliferative disorder of unknown etiology. FNAC (Fine-needle aspiration cytology) is a useful and reliable tool for the diagnosis of RDD, and as such, biopsy is avoidable.

“…Cytology is virtually diagnostic of this self-limited disease. 14 The exact etiology of RDD is not known. Various viruses like Human Herpes virus 6, Epstein -Barr virus and recently, polyoma virus were implicated in the causation, but none were proven.…”

“…Various viruses like Human Herpes virus 6, Epstein -Barr virus and recently, polyoma virus were implicated in the causation, but none were proven. 14 The diagnosis of most inflammatory breast lesions is clinical and usually resolve within a short period. However, rare chronic inflammatory lesions and reactive lesions like RDD with their atypical presentations cause diagnostic dilemmas.…”

“…RDD is a well-defined clinicopathological entity, first described by Destombes in 1965 [ 1 , 5 ]. Later in 1969, Juan Rosai and Ronald Dorfman, recognized it as a distinct disease entity comprising sinus histiocytosis and massive lymphadenopathy [ 5 , 6 ]. 20–50% of RDD patients with nodal/cutaneous disease undergo spontaneous remission [ 2 ].…”

“…4 The prevalence is estimated at 1:200 000, affecting children and young adults most commonly, with an unclear pathogenesis although recent evidence has suggested a viral stimulus, including Epstein Barr virus, HIV, or cytomegalovirus (CMV). 8 Classically, RDD presents nodally, with 43% presenting extranodally, and may present over months to years like other forms of non-LCH. 4 Patients typically have bilateral, massive, painless cervical lymphadenopathy, multisystem involvement, including the skin, bone, and gastrointestine (GI), although CNS involvement is less common.…”
