COVID‐19‐Related Downscaling of In‐Hospital Liver Care Decreased Patient Satisfaction and Increased Liver‐Related Mortality

Abstract: Background&Aims:The COVID-19 pandemic necessitated down-scaling of in-hospital care to prohibit the spread of severe acute respiratory syndrome-coronavirus-2(SARS-CoV-2). We (i)assessed patient perceptions on quality of care by tele-survey(cohort 1) and written questionnaire(cohort 2) and (ii)analyzed trends in elective and non-elective admissions prior to (12/2019- 02/2020) and during (03/2020-05/2020) the COVID-19 pandemic in Austria. Methods:Two-hundred seventy-nine outpatients were recruited into cohort 1 … Show more

“…However, this observation should not be overemphasized, since the concomitant COVID-19 pandemic may have negatively impacted the local cascade of care in 2019-2020, which may have led to reduced referrals. 32 Importantly, we observed 15 HCV reinfections during follow-up corresponding to a reinfection rate of almost 6 per 100-PY. A recent report from the MOSAIC study group in the Netherlands on HIV+ MSM reported even higher reinfection rates of 11.5 per 100-PY, 33 whereas the German NEAT and GECCO studies reported lower rates among HIV+ and HIV-MSM and PWIDs of 1.89 reinfections per 100-PY.…”

“…This might have also contributed to the decline of newly diagnosed AHC infections in 2019–2020, with only 7.0/year after a peak of AHC infections observed in 2017–2018 (17.11/year). However, this observation should not be overemphasized, since the concomitant COVID‐19 pandemic may have negatively impacted the local cascade of care in 2019–2020, which may have led to reduced referrals 32 …”

“…Few data are available about the indirect impact of the COVID-19 epidemic on non-communicable chronic disorders. Excess mortality from cardiovascular diseases ( 22 , 23 ), diseases of the respiratory system ( 22 ), cancer ( 24 , 25 ), and chronic liver disease ( 26 ), possibly due to the disruption of essential health services during pandemic peaks, was observed in some European countries and the USA. Isolation measures have damaged the cognitive and mental health of people with dementia ( 27 ).…”

“…However, enthusiasm for telemedicine must be tempered with caution because downscaling of inhospital liver care decreased patient satisfaction and increased liver-related mortality in one center. 43 In another study, fewer patients with cirrhosis were satisfied with telephone consultations compared with patients with inflammatory bowel disease. 44 Virtual outpatient hepatology is attractive but may not necessarily be the panacea for a sustainable hepatology service.…”
