BK viremia surveillance and outcomes in simultaneous pancreas‐kidney transplant recipients

Abstract: Screening asymptomatic SPK recipients for BKV followed by reduction in maintenance immunosuppression appears to be an effective strategy to prevent kidney allograft dysfunction and graft loss due to BK virus nephropathy, without compromising pancreas allograft outcomes.

“…Fourteen studies included adequate data to calculate the proportion of late BKPyVAN, defined as >1 year post‐transplant (Table ) . Among studies evaluating KTRs, 36.4% of BKPyVAN (27 of 74) occurred late (median 9.1 months post‐transplant, range 1‐113 months).…”

“…An independent analysis of time to diagnosis of BKPyVAN was performed for all evaluable studies. Only those cohort studies in which the proportion of late‐ and early‐onset BKPyVAN could be accurately determined based on the data provided in the published paper were included in Table …”

“…However, it can be removed as an initial therapeutic measure along with the reduction of the CI dose 52 , as the administration of antimetabolite mycophenolate leads to an increase in the incidence of BKV viruria 24 . Even in patients after concomitant renal and pancreatic transplantation, reduction of immunosuppression in BKV viraemia and BKVN seems to be an effective and efficient method of preventing deterioration of renal function, without influencing the prognosis of pancreatic graft 78 .…”

“…The basic treatment strategy is immunosuppressive drug tapering, which is currently the recommended treatment of BKV viremia after kidney transplantation, 5 and using immunosuppressive drugs with antiviral properties are under evaluation. 6 The inhibitors of the mammalian target rapamycin (mTOR) also have data supporting their potential use for BKV infections after transplantation.…”
