Scoping review of cytolytic vaginosis literature

Abstract: Background Cytolytic vaginosis (CV) is a little-known, controversial condition that is typically not considered for women presenting with vulvovaginitis symptoms. Objective: The objective of this scoping review was to identify and compile the global evidence on CV. Methods A medical librarian searched Prospero, Wiley Cochrane Library, Ovid Embase, Ovid Medline, EBSCO CINAHL, ProQuest Dissertations and Theses Global, and Scopus, from inception to April 4, 2019 and updated to October 17, 2021. Studies were eli… Show more

“…In cases with normal microbiota and nonspecific microbial vaginitis, 100% exhibited these characteristics with varying degrees of intensity. A similar condition is found in cytolytic vaginosis (CV), characterized by fragmented epithelial cells and abundant Lactobacilli, which causes an increase in vaginal acidity and epithelial damage [29]. These laboratory findings could help distinguish between CV and candidiasis, as these pathologies do not differ in clinical features [16].…”

“…These laboratory findings could help distinguish between CV and candidiasis, as these pathologies do not differ in clinical features [16]. Despite this, there is still a lack of knowledge about CV and its scope [29]. These processes associated with the pathophysiology of inflammation increase the presence of PMN-generated NETs in the vaginal discharge of women with fungal, bacterial, and parasitic infections, indicating the development of infectious foci at the vaginal level [30].…”

“…(4) It is important to remember the possibility of diagnosing cytolytic vaginosis, another non-infectious alteration resulting from the reduction in vaginal pH. (55) The wet mount vaginal swab has a sensitivity of 50%-60%, depending on the professional's experience in reading the slide. Therefore, common sense and clinical need will guide progress in requesting tests.…”

“…Microscopic investigation reveals abundant lactobacilli and evidence of cytolysis. A connection between CV and L. crispatus overgrowth has been demonstrated [ 43 ]. Clinical manifestation of CV is often similar to that of vulvovaginal candidiasis [ 44 ].…”
