Scope Of Primary Care Physicians’ Participation In The Health Insurance Marketplaces

Abstract: Millions of Americans have purchased health insurance through the Marketplaces, but their access to care is not well understood. Using an audit study, we compared the scope of primary care physicians' participation in Marketplace plans to that in other insurance types in 2016. Across ten diverse states, rates of participation in Marketplace plans were higher than those in Medicaid, but lower than those in employer-sponsored insurance.

“…Findings In this cross-sectional study of 67 057 office-based physicians in 5 states, Medicaid managed care plans included more physicians than health insurance exchange plans in the same geographic markets. Meaning These findings indicate that physicians are likelier to participate in Medicaid physician networks than previously believed, with important implications for the ongoing debate about the role of Medicaid in expanding health insurance and reforming the US health care system.Abstract (continued) suggests that Medicaid remains a viable option for expanding coverage in the United States.Similar to prior work, 18,19 we found large differences across states in the size of the Medicaid and HIX physician networks offered. “…Conversely, the closure of hospital-based obstetric services in rural counties among the United States (US) has increased rates of out-of-hospital and preterm births, and births in hospitals without obstetric units [10]. Also, the scope of primary care physicians varied a lot by health insurance schemes [11].…”

“…A distinctive trend in US health insurance is narrow networks that limit in-network services to a restricted set of clinicians and facilities. [1][2][3][4][5][6][7][8] With frequent churn occurring when patients change insurance plans, [9][10][11][12] insurance network design may have implications for the extent to which churn disrupts the continuity of care. Insurance plan changes decrease the likelihood of establishing a durable primary care relationship, decrease rates of chronic disease control, increase reliance on subspecialists for primary care services, and are associated with greater use of emergency departments.…”
