BackgroundThe International Scientific Society on Scoliosis Orthopaedic and Rehabilitation Treatment (SOSORT), that produced its first Guidelines in 2005, felt the need to revise them and increase their scientific quality. The aim is to offer to all professionals and their patients an evidence-based updated review of the actual evidence on conservative treatment of idiopathic scoliosis (CTIS).MethodsAll types of professionals (specialty physicians, and allied health professionals) engaged in CTIS have been involved together with a methodologist and a patient representative. A review of all the relevant literature and of the existing Guidelines have been performed. Documents, recommendations, and practical approach flow charts have been developed according to a Delphi procedure. A methodological and practical review has been made, and a final Consensus Session was held during the 2011 Barcelona SOSORT Meeting.ResultsThe contents of the document are: methodology; generalities on idiopathic scoliosis; approach to CTIS in different patients, with practical flow-charts; literature review and recommendations on assessment, bracing, physiotherapy, Physiotherapeutic Specific Exercises (PSE) and other CTIS. Sixty-five recommendations have been given, divided in the following topics: Bracing (20 recommendations), PSE to prevent scoliosis progression during growth (8), PSE during brace treatment and surgical therapy (5), Other conservative treatments (3), Respiratory function and exercises (3), Sports activities (6), Assessment (20). No recommendations reached a Strength of Evidence level I; 2 were level II; 7 level III; and 20 level IV; through the Consensus procedure 26 reached level V and 10 level VI. The Strength of Recommendations was Grade A for 13, B for 49 and C for 3; none had grade D.ConclusionThese Guidelines have been a big effort of SOSORT to paint the actual situation of CTIS, starting from the evidence, and filling all the gray areas using a scientific method. According to results, it is possible to understand the lack of research in general on CTIS. SOSORT invites researchers to join, and clinicians to develop good research strategies to allow in the future to support or refute these recommendations according to new and stronger evidence.

e archivio di dimensionale comparsa durante l'accrescimento: nel piano frontale si evidenzia uno spostamento delle vertebre dall'asse mediano. La vertebra più distante dall'asse mediano viene denominata vertebra apicale; l'angolo di Cobb viene misurato fra il piatto superiore della vertebra limitante superiore (VLS) e il piatto inferiore della vertebra limitante inferiore (VLI). Nel piano orizzontale si evidenzia la rotazione dei corpi vertebrali verso la convessità della curva: clinicamente questa rotazione si manifesta con una gibbosità e sulla radiografia viene misurata a livello della vertebra apicale, valutando la posizione del peduncolo e della spinosa in rapporto all'immagine del corpo vertebrale. Nel piano sagittale si verifica una riduzione della fisiologica lordosi lombare e una evoluzione progressiva in cifosi. In una deviazione scoliotica lombare si possono individuare: • la zona apicale, formata da 2 o 3 vertebre (Fig. 2) la controcurva lombo-sacrale, situata al di sotto della VLI con i dischi pinzati dal lato della convessi-• la frequenza aumenti con l'età, soprattutto dopo i 60 anni • la frequenza sia dell'ordine del 10% all'età di 65 anni • la predominanza nel sesso femminile diminuisca con l'aumento dell'età [1,2]. Per quanto concerne l'eziologia, escludendo le scoliosi a eziologia particolare associate a una patologia primitiva o secondaria del rachide (affezioni neuro-muscolari, forme congenite o acquisite), possiamo distinguere: • scoliosi lombari idiopatiche comparse durante l'infanzia o l'adolescenza, trattate o non trattate, che persistono in età adulta un numero imprecisato di scoliosi minori passate inosservate sino a che un aggravamento spontaneo, con le relative conseguenze funzionali, non le ha evidenziate • scoliosi lombari comparse in età adulta in cui le radiografie a fine accrescimento mostravano un rachide lombare perfettamente rettilineo e in cui radiografie successive hanno evidenziato un'evoluzione spesso rapida. Queste scoliosi "de novo" sono in realtà scoliosi artrosiche.Valutazione clinica e radiografica L'esame clinico e le radiografie standard in AP e nella proiezione laterale evidenziano le componenti di questa deformazione tri- IntroduzioneL'evoluzione delle scoliosi idiopatiche in età adulta è dimostrata e giustifica una sorveglianza regolare dopo il termine dell'accrescimento scheletrico. Spesso l'evoluzione durante l'età adulta è in cifosi, per cui si rende necessaria una sorveglianza con radiografie di fronte e di profilo soprattutto durante il periodo della menopausa. Le curve lombari, che sono le meno evolutive durante la pubertà, sono quelle che presentano la maggiore evoluzione nell'età adulta. L'aggravamento durante l'età adulta dipende soprattutto da due tipi di fattori: meccanici e metabolici. Tra i fattori meccanici (discopatie e artrosi) annoveriamo il pinzamento discale asimmetrico, la distensione capsulo-legamentosa, l'artrosi delle articolari, la deformazione progressiva dei corpi vertebrali e degli archi posteriori. Les causes et les cons6quences de la scoliose Les causesLa scoliose peut exister d6s l'adolescence et s'aggraver avec le temps surtout chez la femme apr~s la m6nopause. Mais la scoliose peut aussi survenir h l'~ge adulte pour des causes purement ddg6n6ratives. L'arthrose attaque l'articulation vert6brale post6rieure. On assiste 7t une hypertrophie des massifs articulaires et h une usure des surfaces articulaires. Celles-ci laissent alors un degr6 de mobilit6 inter-vert6brale. Parall~lement, le disque se d6g6nbre. Lui qui absorbe un maximum des efforts en torsion [3]va autoriser une instabilit6 par d6verrouillage intersomatique. Cette instabilit6 ant6rieure et post6rieure va autoriser un d6place-ment des vert~bres en rotation. Cette rotation inter-vert6brale aboutit h une torsion de la colonne lombaire [8] avec sa d6formation bien connue en scoliose qui s'accompagne, ~ cet 6tage, d'une composante ant6ro-post6rieure en cyphose ( fig. 1).
