Sclerosing Odontogenic Carcinoma with Local Recurrence: Case Report and Review of Literature

Abstract: Sclerosing odontogenic carcinoma is a rare locally destructive neoplasm with many histologic mimics. Here the diagnostic challenges are presented of a case of sclerosing odontogenic carcinoma with variable histologic features, including unusual and unexpected negative immunostaining for CK19.

“…CK19 is a low molecular weight cytokeratin and often highlights epithelial cells near the surface epithelium or squamous differentiation. 23 This marker, however, was unexpectedly negative in the case reported by Todorovic et al 7 More clarifications of cytokeratin subtype staining would be beneficial to identify the expression of cytokeratin pertaining to its histogenesis. Predictive biomarkers such as Ki-67 that are widely used to gauge the proliferative index of a tumor appeared to be a non-significant finding for SOC.…”

“…This may be related to the low-grade characteristics of epithelial and stromal components, including their low mitotic activity. 7 Saxena et al 25 also postulated that the dense stroma surrounding the epithelial component of the tumor may have a role in preventing metastasis. The role of adjuvant treatment cannot be justified at the moment.…”

“…Depression on the palatal gingiva attributed to bone perforation and collapse was seen in one case of SOC 12 and some cases of COdF, 13 whilst neurological signs were absent occasionally in SOC 6, 14 and usually in COdF 13 . Presentation as a destructive osteolytic lesion is common in SOC 15 and large COdFs of the maxilla 13, 16 …”

“…Clinically, SOC is an expansile mass that sometimes results in nerve symptoms. It is associated with osteolysis, variable cortical destruction, sclerotic changes, ground-glass appearance of the involved bone, a wholly or partially ill-defined border, and tooth resorption evident on panoramic x-ray [ 19 , 20 ]. In the current case, the border of the lesion was clear, there were no sclerotic changes or clinical symptoms.…”
