Sclerosing encapsulating peritonitis: three case reports and review of the literature

Abstract: Sclerosing encapsulating peritonitis (SEP) is a disease that is rarely encountered clinically. Preoperative diagnosis of SEP can be difficult. However, with imaging technology, such as computed tomography (CT), this condition can be diagnosed without surgery and pathological analysis. SEP is characterized by small intestine being partially or completely encased by a layer of a thick grayish-white fibrocollagenous membrane similar to a cocoon. The most common symptoms of SEP are abdominal pain, nausea, and vomi… Show more

“…[ 2 ] Because of the lack of specificity in the early clinical manifestations, preoperative diagnosis of SEP is difficult. [ 46 ] A high index of clinical suspicion is mandatory. Apart from patient’s history and clinical presentation, imaging techniques play an important role in diagnosis of SEP. Diffuse air-fluid levels and dilated small intestine loops may be seen on abdominal X-rays.…”

“…As in the present case, the imaging appearance of SEP is visualized as the central accumulation of the small bowel encased by a thin membrane and loculated fluid collections. Other lesions such as ascites, intestinal obstruction, abdominal calcifications, and lymphadenopathy may be present [ 6 ].…”

“…The aforementioned theories are well supported by recent studies speculating on the diversity of the biochemical and metabolic profiles of the peritoneal fluid in patients with endometriosis. More specifically, according to Polak et al, the hemoglobin levels in the peritoneal fluid of patients with endometriosis were significantly elevated compared to both controls and women with ovarian cysts, while, interestingly, antioxidant parameters were significantly lower in patients with endometriosis, creating an oxidative intraperitoneal environment [56][57][58].…”
