Sclera as the Target Tissue in Progressive Myopia

Abstract: Wstęp: Celem pracy było zbadanie osłony rogówkowo-twardówkowej i systemu tkanki łącznej (CT) u dzieci z nabytą oraz wrodzoną krótkowzrocznością postępującą, a także zidentyfikowanie informacyjnych kryteriów diagnostycznych wspomagających funkcję twardówki.Materiał i metody: Przebadano 155 dzieci w wieku 9–17 lat. 18 miało emetropię lub hiperopię, 20 miało niską krótkowzroczność, 32 średnią i 85 wysoką krótkowzroczność. 32 dzieci miało krótkowzroczność powikłaną (CM) z powodu obwodowego zwyrodnienia siatkówki. … Show more

“…The presented results confirm that the sclera may indeed be of fundamental importance in myopia progression. [3][4][5] This may be visualized by considering the emmetropic sclera as a half-inflated balloon. When not fully inflated certain shape irregularities may still be seen in the balloon surface.…”

“…20 Similarly, a recent work on schoolage children with progressive myopia reported biomechanical abnormalities of the corneo-scleral area. 4 As the sclera affects the restraint or facilitation of eye growth, different myopia control strategies have been proposed based on strengthening the sclera, such as scleral reinforcement and collagen cross-linking. 3 However, most current techniques for myopia control are highly invasive and show a variable outcome between patients.…”

“…Studies show that an increased level of cortisol inhibits collagen synthesis in connective tissues and isolated fibroblasts ( 65 ). Shifts in hormone levels cause deregulation between matrix metalloproteinases and tissue inhibitors of metalloproteinases (TIMs), which disrupt cross linking of fibrils and fibers and reduce the transverse crosslinking of the collagen network in the sclera ( 66 ), leading to sclera degradation [ Figure 2A(c )].…”

“…The major manifestations of CM include increased joint mobility, softness of joint capsules and ligaments (Iomdina et al, 2015). Other symptoms including ease of dislocations and joint sprains, muscle and joint pain, hyperelastic skin, vision disorders, disorders of the gut, problems with body stabilization and other Brighton criteria indicate the presence of BHJS.…”
