Die Ersatzoperation des vorderenKreuzbands in der Bone-tendon-boneTechnik mittels des mittleren freien Patellarsehnendrittels führt nicht in allen Fällen zu einem stabilen Kniegelenk. Bei Nachuntersuchungen größerer Patientenkollektive fiel auf, daß eine Reihe von Patienten eine Restinstabilität behielten. Diese Beobachtungen werden von anderen Autoren großer Nachuntersuchungskollektive bestä-tigt. Übereinkunft besteht mittlerweile bei der Festlegung der Isometriepunkte [12,13]. Kontroversen existieren jedoch hinsichtlich der Transplantatwahl und der Fixationstechnik.Die Ursachen für ein sog. Transplantatversagen sind mannigfaltig. Technische Aspekte, wie eine gelenkferne Fixation des femoralen und/oder tibialen Knochenblocks sowie der Bungee-Effekt werden als Ursachen einer Transplantatinsuffizienz angeschuldigt. Des weiteren fanden sich radiologische Phänomene, wie der Scheibenwischereffekt. Dieser ist je nach Fixationsmethode femoral und/ oder tibial zu beobachten und Zeichen einer Relativbewegung des Transplantats im Bohrkanal. Er korreliert jedoch nicht unbedingt mit einer vermehrten klinischen Instabilität [15].Parapatellare Schmerzen und andere mit der Entnahme des mittleren Patellarsehnendrittels vergesellschaftete Probleme rücken immer mehr in den Vordergrund der Diskussion um die geeignete Transplantatwahl und die geeignete Fixations-und Operationstechnik.Auch bei der Kreuzbandersatzoperation mit der Semitendinosussehne in der Endobutton ® -Technik ergeben sich bezüglich der Restinstabilität und des Scheibenwischereffekts erhebliche Probleme. Bei der Endobutton ® -Fixation kommt zusätzlich noch das Phä-nomen des Bungee-Effekts hinzu. Es handelt sich hierbei um ein Phänomen, das aufgrund der Dehnbarkeit der Semitendinosussehne und der Fixationsmaterialien auftritt. Wie alle Sehnen weist die Semitendinosussehne eine Dehnbarkeit von bis zu 10% in Abhän-gigkeit von der Vorspannung auf. Durch die zusätzliche Dehnbarkeit der Fixationsbänder bzw. der Tapes (geflochtenes, nicht resorbierbares Material) kann es zu einer Transplantatelongation und konsekutiver Instabilität des Kniegelenks kommen. Der Ausdruck Transplantatelongation ist hier nicht ganz korrekt, da es sich sowohl um eine Dehnung des Implantats als auch des Fixationskonstrukts handelt, wobei der überwiegende Anteil auf Kosten des Fixationsmaterials geht.Aufgrund dieser Beobachtungen hielten wir in unserer Klinik einen Wechsel von der BTB-Methode zur Endobutton ® -Fixation für nicht indiziert, da wir hier bezüglich der Primär-stabilität der Fixation sowie des Transplantats und des somit zu erwartenden klinischen Outcomes keine besseren Arthroskopie (1999) 12 : 305-312 Zusammenfassung In den letzten Jahren wird die Kreuzbandersatzoperation mit dem mittleren Patellarsehnendrittel zunehmend mehr in Frage gestellt. Gründe sind die hohe Transplantatentnahmemorbidität sowie Probleme bei der gelenknahen Fixation. Der Scheibenwischereffekt, der Bungee-Effekt u. a. seien hierfür stellvertretend genannt.
