Scaling Up Human Immunodeficiency Virus Screening and Antiretroviral Therapy Among Men Who Have Sex With Men to Achieve the 90-90-90 Targets in China

Abstract: Achieving the 90-90-90 targets by 2020 requires steep increase in investment, but delaying the targets to 2025 is practical and cost-effective.

“…A free VL testing was provided to patients once a year, who had received ART for more than 6 months, but 11.8% HIV‐1 infected patients on ART had not conducted VL testing in 2015 6 . A compartmental model was constructed to predict the HIV epidemic in Chinese men who have sex with men (MSM), HIV prevalence would increase from 9.2% to 12.6% during 2016 to 2020, only 49.2% infected MSM would be diagnosed by 2020, to achieve 90‐90‐90 targets by 2020, an extra 850 000 HIV screening tests were needed from 2016 to 2020 7 . To achieve the UNAIDS targets, more feasible HIV VL monitoring approaches are desirable.…”

“…Although the national HIV prevalence has remained low (< 0.1%) in China, this prevalence has been increasing among men who have sex with men (MSM) [1]. HIV prevalence among MSM increased from 1.5% in 2005 to 8.0% in 2015 [2, 3].…”

“…It is estimated that 1 out of 70 men could be MSM. If the current HIV epidemic in MSM were not contained, the overall HIV prevalence would increase from 9.2% in 2016 to 12.6% in 2020 and 16.2% in 2025, [1]. Condom use is considered to be one of the most effective intervention strategies [4].…”

“…Globally, young men who have sex with men (YMSM) represent one of the most at-risk groups for new HIV infection (Beyrer et al, 2012; World Health Organization, 2015), and in China particularly, the HIV epidemic among YMSM is increasing rapidly (Li, Jiang, & Zhang, 2019; Wu, 2018). According to data released by the Chinese Center of Disease Prevention and Control (China CDC), HIV prevalence among MSM had increased from 0.9% in 2003 to 8.0% in 2015 (China National Health and Family Planning Commission, 2016), and if these trends continue unabated, experts estimate that one in six Chinese MSM will be infected by HIV in 2025 (Zhuang et al, 2018). As is true for at-risk MSM globally, condomless anal sex (CAS) represents the primary route of HIV transmission among Chinese YMSM (Li et al, 2019; Wu, 2018; Zhang, Chow, & Wilson, 2012); preexposure prophylaxis (PrEP) has not been adopted in China (Han et al, 2019).…”
