As the coronavirus pandemic is far from ending, more questions regarding the female reproductive system, particularly fertility issues, arise. The purpose of this paper is to bring light upon the possible link between COVID-19 and women’s reproductive health. This review emphasizes the effect of SARS-CoV-2 on the hormones, endometrium and menstrual cycle, ovarian reserve, follicular fluid, oocytes, and embryos. The results showed that endometrial samples did not express SARS-CoV-2 RNA. Regarding the menstrual cycle, there is a large range of alterations, but they were all reversible within the following months. The ovarian reserve was not significantly affected in patients recovering from both mild and severe infection in most cases, except one, where the levels of AMH were significantly lower and basal follicle-stimulating hormone (FSH) levels were increased. All COVID-19 recovered patients had positive levels of SARS-CoV-2 IgG in the follicular fluid. The amount of retrieved and mature oocytes and the fertilization rate were unharmed in three studies, except for one study, where the quantity of retrieved and mature oocytes was reduced in patients with higher levels of SARS-CoV-2 antibodies. The numbers of blastocysts, top-quality embryos, and euploid embryos were affected in most of the studies reviewed.

ObjectivesTo investigate the relationship between prior coronavirus disease 2019 (COVID‐19) infection in females and subsequent treatment outcomes of assisted reproductive technology (ART).MethodsA systematic literature review was carried out up to 16 December 2022, in PubMed, Web of Science, EMBASE, and Cochrane Library. Random‐effect models were adopted to estimate the pooled effects as mean differences (MDs) or odds ratios (ORs). I2 statistic and Egger's test were applied to assess heterogeneity and publication bias, respectively.ResultsAfter screening 1480 records, 15 cohort studies totalling 1905 cycles were included in this meta‐analysis. In a comparison of previously COVID‐19‐infected versus uninfected women, no significant differences were observed in the primary outcomes of the retrieved oocytes number (MD = 0.06; 95% confidence interval [CI]: ‐0.75–0.88; I2 = 0) and clinical pregnancy rate (OR = 0.96; 95% CI: 0.74–1.24; I2 = 0). Pooled analyses of other predefined outcomes, which encompassed four cycle characteristics, six laboratory indicators and four pregnancy results, also showed no adverse effects of prior COVID‐19 infection. Most outcomes remained consistent after further sensitivity and subgroup analyses, and no significant publication bias was observed.ConclusionsOur work provides the first systematic evidence that COVID‐19 infection history in females may have no measurable detrimental impact on the subsequent ART cycle. More data are needed to assess the live birth outcome and the optimal time interval from infection to assisted reproduction.

Coronavirus disease 2019 (COVID‐19) has experienced a global pandemic, and currently, the emergence of its variants has posed challenges in terms of prevention and treatment. Nonetheless, the effect of COVID‐19 infection on female reproductive function is unclear. This study aimed to systematically evaluate for the first time the causal effect of COVID‐19 on female reproductive function. Genetic correlations were assessed using linkage disequilibrium score regression. Mendelian randomization (MR) analysis was performed using summary statistics of two variables, including COVID‐19 severity and eight female reproductive traits. The three degrees of severity had genetically significant associations with sex hormone‐binding globulin (SHBG) concentrations (rg = –0.153, p = 0.004; rg = –0.187, p < 0.001; rg = –0.180, p = 0.003). Additionally, MR showed that SHBG (β = –0.020, p = 0.040) and total testosterone levels (β = –0.061, p = 0.009) followed a decreasing trend, as the COVID‐19 infection higher. No significant genetic association was found between COVID‐19 infection and total estradiol concentrations, menstruation, and female infertility. Simultaneously, MR found no causal relationships between COVID‐19 infection and total estradiol concentrations, menstruation, and female infertility (all p > 0.05). In conclusion, COVID‐19 was causally associated with lower SHBG and total testosterone concentrations, offering invaluable insights that will help guide clinical decision‐making.
