Sarcopenia as a Predictive Factor for Response to Upfront Cisplatin-Based Chemotherapy in Patients with Muscle-Invasive Urothelial Bladder Cancer

Abstract: Purpose: We hypothesized sarcopenia as predictive factor as a response to upfront chemotherapy of muscle-invasive urothelial bladder cancer (MIBC). Methods: We retrospectively studied 30 patients who received upfront cisplatin-based chemotherapy for MIBC (pT2–4 N0/+ M0) before planned radical cystectomy. Skeletal muscle index (SMI) was calculated by CT at study baseline and following completion of chemotherapy. Patients were stratified according to the presence of sarcopenia. Endpoints included clinical and pa… Show more

“…The preoperative staging was not standardized and we had no information on preoperative comorbidities assessed by a validated score. Moreover, variability in standard practices, quality of surgery, frequency of surveillance imaging, sarcopenia and nutritional status in the elderly [41], as well as other selection biases and other not measurable confounders, may have influenced our results.…”

“…15,16 Furthermore, NAC has been shown to cause a measurable decrease in SMI of 3 -8.4%. 17,18 In our study, we hypothesized that NAC is associated with increased chances of a decrease in SMI, and that sarcopenic patients would have a higher number and grade of adverse events associated with NAC (prior to RC).…”

“…Among advanced UC patients treated with ICI, the ORR was significantly higher in those without sarcopenia compared to those with sarcopenia ( p = 0.019) [ 31 ]. However, according to Klatte et al, there was no association between pre-NAC sarcopenia and tumor response ( p = 0.65) in muscle-invasive bladder cancer patients [ 32 ].…”
