Sarcomatoid squamous cell carcinoma of uterine cervix: Pathology, imaging, and treatment

Abstract: Sarcomatoid squamous cell carcinoma of the cervix is a rare tumor. Only 16 cases have so far been reported in literature. We report here one such tumor occurring in a 54-year-old postmenopausal woman. Our case report describes the clinical, pathological, and PET scan characteristics of this tumor. The patient was treated with concurrent chemoradiotherapy and is disease free at 6-months follow-up.

“…It comprises only 1-2% of all gynecological malignancies [7]. A comprehensive literature review using the terms “sarcomatoid carcinoma” and “squamous cell carcinoma” identified 20 reported cases, including a case series of nine patients (Brown et al) [46, 810]. As with the more common types of cervical cancer, HPV is the primary etiologic factor identified to date; high risk subtypes 16 and 18 have been found in both squamous and sarcomatoid components of the tumor [11].…”

“…It is necessary to consider sarcomatoid squamous cell carcinoma as a possible differential diagnosis. Before epithelial-mesenchymal transition theory, i.e., the theory that carcinoma transforms into sarcoma, was widely accepted, there was some confusion about differentiating between the two diseases [ 12 ]. The traditional view was that carcinosarcoma does not exhibit merging of its carcinomatous and sarcomatous components.…”

“…In an English literature search for cervical sarcomatoid carcinomas, around 18 cases were found. [123] SCC has been described more frequently in the oral cavity, esophagus, pharynx, larynx and skin. Brown et al in their study have reported the largest series of 9 cases of sarcomatoid carcinomas of the cervix with a median disease-free interval of 4.9 months (2-9.5 months).…”

“…Until date, only 18 cases of sarcomatoid carcinoma of the cervix have been reported in the literature. [123] The prognosis for women with cervical sarcomatoid carcinomas tends to be worse than that of squamous histology. Patients often present with more advanced stages and follow a very aggressive course of the disease.…”
