Sandwich osteotomy of the atrophic posterior mandible prior to implant placement: A case report

Abstract: SUMMARYThe aim of this report was to present a patient with atrophic posterior mandible, rehabilitated with endosteal dental implants after a sandwich osteotomy of the mandible. Some aspects of the procedure are discussed as well as its outcome.

“…Excessive resorption of the alveolar ridge in a vertical direction may compromise possibility of implant placement. This will eventually result in insufficient bone superior to the inferior alveolar nerve (IAN), making dental implant placement impossible without performing augmentation of the alveolar bone in terms of height 2 .…”

“…Bone grafts for horizontal bone atrophy have high rates of success, with survival rates ranging from 90.2% to 100% after 3 years of evaluation when autologous bone grafts were used. 1,2 In cases of vertical bone atrophy, different techniques may be used including guided bone regeneration (GBR) with titanium mesh or membranes reinforced with titanium, 3 intraoral distractors, 4,5 the bilaminar cortical tenting grafting technique, 6 the tent-pole technique using screws or implants, 7 the sandwich osteotomy, 8 and so forth. In all of the previously cited techniques, bone regeneration is usually realized before the installation of the dental implant, which requires longer treatment durations and high financial costs.…”

“…In all of the previously cited techniques, bone regeneration is usually realized before the installation of the dental implant, which requires longer treatment durations and high financial costs. [8][9][10] The use of a nonabsorbable and dimensionally stable barrier is usually necessary to prevent the collapse of the grafting material. 3 A limitation is the amount of soft tissue available for covering the graft material to prevent early exposure of the membrane.…”
