Samhandling og informasjonsflyt når eldre flytter mellom ulike helse- og omsorgstilbud i kommunen

Abstract: Bakgrunn: En økende spesialisering i kommunale helse-og omsorgstjenester har ført til at eldre i større grad beveger seg mellom ulike tilbud i kommunen. For å ivareta pasientsikkerheten er informasjonsoverføring mellom tjenestene viktig.
