Salvage Surgery for Cervical Cancer Recurrences

Abstract: Cervical cancer usually presents in advanced stages and is treated with chemoradiation. About 15-20 % patients present with local recurrence after chemoradiation. Radical surgical resection is the only treatment modality offering long term survival benefit in recurrent cervical cancer. The most common surgical option for these patients is pelvic exenteration. Radical hysterectomy may be done for patients with a small centrally located recurrence in the cervix with no infiltration of adjacent structures. The ai… Show more

“…Radical hysterectomy is an option for localized central recurrence limited to cervix or vaginal orifices. [ 8 9 ] The present study was unique as it catered to patients with both early and recurrent operable cervical cancer, and survival data were calculated. Patients with early disease with no risk factors have better outcomes than patients with high-risk factors and recurrent cervical cancer.…”

“…Inaccurate contouring at this site therefore may result in a geographical miss or an increase rate of late vaginal toxicity. Following definitive radiotherapy the rate of isolated vaginal recurrence is very low, 18,19 however, these are associated with significant morbidity and mortality. If the lower boundary is extended too far inferiorly then it may result in increased rates of toxicity such as vaginal fibrosis and stenosis.…”

“…In our study, four studies performed salvage surgery after confirmation of residual disease on biopsy. Three studies confirmed the presence of residual disease in 100% of the salvage surgery pathology specimens 7,21,23 . The study by Boers et al confirmed the presence of residual disease in salvage surgery specimens in 72% of women 22 .…”
