Introdução: O adenoma pleomórfico, uma neoplasia benigna, é uma das patologias mais comuns de glândula salivar, cujo tratamento é sua excisão cirúrgica. O objetivo deste estudo é relatar um caso de adenoma pleomórfico de grande proporção em glândula salivar acessória na região do palato duro. Relato de caso: Paciente do sexo feminino, 22 anos, encaminhada de sua unidade básica de saúde devido a uma lesão em região de palato, assintomática, com evolução de 1 ano. No exame clínico intra-bucal observou-se um nódulo extenso em palato do lado direito. A biópsia incisional revelou o diagnóstico de adenoma pleomórfico. A paciente foi submetida a excisão cirúrgica da lesão sob anestesia geral. Apresentou boa evolução pós-operatória, sem recidivas. Discussão: O Adenoma Pleomórfico é uma neoplasia de diagnóstico clínico, com confirmação a partir de exame anatomopatológico. Embora seja uma patologia de crescimento lento, quando o tratamento não é realizado de maneira precoce, ele pode atingir grandes dimensões, o que leva a um aumento da morbidade do procedimento cirúrgico e pode levar a maiores taxas de recidiva e/ou transformação maligna. Conclusão: Salientamos a importância de um diagnóstico preciso e um tratamento efetivo com a maior brevidade. Quando possível, as próteses obturadoras/aparelhos intraorais acrílicos podem e devem ser utilizados de forma associada aos tratamentos cirúrgicos de tumores; assim, restabelecendo função e dando conforto no pós-operatório.

Background: Salivary gland tumors (SGT) are uncommon tumors of diverse histopathology accounting for <2% of all neoplasms. Early diagnosis differentiates between benign and malignant SGT and less extensive surgical procedure can be performed. The objective is to study the spectrum of primary SGT with special emphasis on their distribution, treatment and outcome at a rural tertiary care centre.Methods: A prospective observational study was conducted on 30 consecutive primary SGT patients attending the Departments of Surgery and Otorhinolaryngology of North Bengal Medical College and Hospital over a period of one and half years. All patients were clinically evaluated, investigated, treated accordingly and followed up during the period of study. Data was collected and compiled in Excel sheet and analysed using GraphPad Software and GraphPad QuickCalcs 2018 (San Diego, CA). A p value <0.05 was considered statistically significant.Results: Maximum patients (83.3%) were in the age range of 31-60 years. The male to female ratio was 1:4 for benign tumours and 1:1 for malignant tumours. Malignant cases were 66.67%. Involvement of parotid gland was the commonest (43.3%) and mostly presented with swelling (73.3%). Pleomorphic salivary adenoma was the commonest benign tumor (33.3%) and mucoepidermoid carcinoma was the commonest malignant tumor (16.7%). Permanent facial palsy was observed in 2 (9.5%) and recurrence in 4 patients (19%).Conclusions: SGT are rare and present in various modes. Malignant cases were singularly more in this study.

A nyálmirigydaganatok nem sorolhatók a gyakori tumoros megbetegedések közé. Valószínűleg éppen ezért előfordulásukat tekintve világszerte viszonylag kevés szakirodalmi adat áll rendelkezésünkre. Cikkünkben igyekeztünk ezeketa tanulmányokat úgy összegyűjteni a különböző kontinensekről, hogy egy összefoglaló, releváns képet kaphassunk epidemiológiájukról. Kutatásunkhoz a PubMeden kerestünk publikációkat 1998 és 2018 között, és a kapott eredményeket rendszereztük. A benignus daganatok magasabb arányban, átlagosan 65,6%-ban fordultak elő, a malignusak 34,4%-ban, kivéve Nigériát és Iránt. Itt a malignus daganatok voltak gyakoribbak. Az átlagos életkor 46,8 év, Európában az 50-es, egyéb kontinenseken a 40-es korosztály érintett. A nemek közötti megoszlásban kiegyenlítettség található, kivéve Mexikót, Kamerunt és Teheránt, ahol jelentős a női dominancia. A legtöbb tumor a parotisban lokalizálódott, a leggyakoribb jóindulatú tumor a pleomorph adenoma, míg a rosszindulatúak között az adenocysticus carcinoma és a mucoepidermoid carcinoma álltak az első helyen. A világ egyes országaiban észlelt eltérések egyrészt földrajzi, etnikai és szociális okokra, másrészt az egészségügyi adminisztráció, regisztráció különbözőségére vezethetők vissza. Az okok pontosabb feltárására további vizsgálatokra van szükség.
