Salivary gland pleomorphic adenoma in the Netherlands: A nationwide observational study of primary tumor incidence, malignant transformation, recurrence, and risk factors for recurrence

Abstract: This first nationwide study clearly showed sex differences in SGPA epidemiology, possibly suggesting some underlying hormonal mechanism. Long-term recurrence risks were low, and secondary malignant transformation risks were very low.

“…SGN are an uncommon heterogeneous group of lesions, with varying biological features, arising within tissue of the major or minor salivary glands (El‐Naggar, Chan, Grandis, Takata, & Slootweg, ). There is a dearth of information on the total incidence of SGN in various regions due to underreporting of cases in national cancer registries (Valstar et al, ).…”

“…Data regarding the malignant transformation potential of a pleomorphic adenoma ranged from 1.1% to 6.2% . The progression of pleomorphic adenoma to CXPA involves a diversified process comprising genomic instability and successive mutations during the neoplastic process development, especially the particular loss of tumour suppressor genes, as well as the presence of epigenetic alterations .…”

“…The standard treatment for PA of the parotid gland is resection of the tumour with the surrounding salivary gland tissue and preservation of the facial nerve. PA recurrence is diagnosed in approximately 3% of patients [11]. The method of PA extracapsular enucleation is not widely accepted due to the significantly higher relapse rate of 45% [12].…”

“…The use of postoperative radiotherapy in the case of recurrent PA is controversial. According to some authors it prevents further recurrences, while according to others it creates the possibility of malignant transformation [1, 11,12].…”
