Safety of the Pipeline Embolization Device in Treatment of Posterior Circulation Aneurysms

Abstract: BACKGROUND AND PURPOSE:The published results of treating internal carotid artery aneurysms with the PED do not necessarily apply to its use in the posterior circulation because disabling brain stem infarcts can be caused by occlusion of a single perforator. In this multicenter study, we assessed the safety of PED placement in the posterior circulation.

“…Phillips et al treated 32 posterior circulation aneurysms with the PED but reported perforator infarctions in 14%, with a permanent morbidity of 9.4%. 16 In this current series, only 1 case (a symptomatic cavernous aneurysm) of a large perforating vessel was seen to be occluded (ophthalmic artery) at angiographic follow-up after initially filling immediately post-stenting. The patient was not symptomatic, and at follow-up the collaterals from the external carotid artery continued to perfuse the orbital contents; the patient's vision remained normal.…”

“…We noted that in a great majority of single-center large series concerning stenting for intracranial aneurysms, an age range to include children was present (12,26,27). However, no information specific to the sizing of the devices or antiplatelet drug administration in children was provided in these series.…”

“…Of the total of 32 aneurysms in the posterior circulation in the article by Phillips et al, 27 only 8 were located in the basilar trunk. No detailed information on aneurysm morphology or giant subtype was provided.…”

“…Data available in the literature on the use of flow-diverting stents, mostly case series, gave rise to optimism. In a series by Phillips et al, 27 32 aneurysms of the posterior circulation were treated with flow-diverting stents. Mean aneurysm size was 9.7 mm.…”
