Objective Carotid artery stenosis in patients undergoing open-heart surgery may increase risk and deteriorate outcome. The aim of the study was the analysis of risks and outcome after simultaneous carotid and cardiac surgery. Methods We retrospectively reviewed the medical records of 100 consecutive patients who underwent simultaneous carotid surgery and open-heart surgery during a 5-year period (from 2006 to 2010). Seventy patients were male and 30 female; the mean age was 70.9±7.9 years (median: 71.8 years). Seventy-three patients underwent coronary bypass grafting (CABG), 18 patients combined CABG and valve procedures, 7 patients CABG combined with other procedures, and 3 patients isolated valve surgery. More than half of patients had had bilateral carotid artery pathology (n=51) including contralateral carotid artery occlusion in 12 cases. Results Carotid artery patch plasty was performed in 71 patients and eversion technique in 29. In 75 cases an intraluminal shunt was used. Thirty-day mortality rate was 7% due to cardiac complications (n=5), metabolic disturbance (n=1), and diffuse cerebral embolism (n=1). There were no carotid surgery-related deaths. Postoperatively, transient cerebral ischemia occurred in one patient and stroke with mild permanent neurological deficit (Rankin level 2) in another patient. Conclusion Simultaneous carotid artery surgery and open-heart surgery have low risk. The underlying cardiac disease influences outcome.

To determine the effectiveness of simultaneous surgical interventions for revascularization of the carotid and coronary basins. Materials and methods: The effectiveness of the surgical intervention was assessed in 34 patients (31 male patients, 3 female patients, average age 61.8 ± 1.52 years) who underwent simultaneous carotid endarterectomy and coronary artery bypass grafting. The indications for surgical treatment for carotid arteries disease were determined by a neurosurgeon, and for coronary artery bypass grafting-by a cardiac surgeon. Results: Postoperative complications were observed in 55.9 % of patients. In 3 (8.8 %) patients, an acute ischemic stroke with a neurological deficit developed on the first day after the operation. One patient (2.9 %) presented with both an acute myocardial infarction in the postoperative period and an acute heart failure. Perioperative rhythm disturbances in the form of atrial fibrillation were observed in 6 (17.6 %) cases. Postoperative mortality was 2.9 %. During the first year after surgery, an improvement in the quality of life was observed

Цель исследования-изучить нейрофизиологический статус пациентов, перенесших одномоментное коронарное шунтирование (КШ) и каротидную эндартерэктомию (КЭЭ), по данным многоканальной компьютерной электроэнцефалографии (ЭЭГ). Материал и методы. В исследование включены 44 пациента. Сочетанные КШ и КЭЭ выполнены у 17 больных, изолированное КШ проведено 27 пациентам, у которых отсутствовали значимые стенозы внутренних сонных артерий (ВСА). В группе КШ+КЭЭ степень сужения ВСА составляла 70-99%. Поражение правой ВСА отмечено у 9 (53%) больных. Всем пациентам проводили ЭЭГ за 3-5 дней до и на 7-10-е сутки после вмешательств. Анализ данных выполнялся с помощью статистического пакета Statistica 8 («StatSoft», Tulsa, OK, США). Результаты. Установлено, что как сочетанные вмешательства, так и изолированное КШ сопровождались диффузным увеличением мощности биопотенциалов θ-ритмов по сравнению с предоперационными показателями, а также снижением мощности биопотенциалов α 1-ритма в задних регионах коры на 7-10-е сутки после операции. Особенностью группы КШ + КЭЭ был преимущественный рост мощности биопотенциалов θ 2-ритма в правом полушарии по сравнению с левым. Заключение. В раннем послеоперационном периоде после сочетанного КШ и КЭЭ наблюдается сопоставимый уровень мощности биопотенциалов тета-ритмов по сравнению с изолированным КШ. Компьютерная ЭЭГ может являться высокочувствительным методом, позволяющим проводить динамическую оценку нейрофизиологического статуса пациентов и безопасности операции. Ключевые слова: стенозы сонных артерий, сочетанное вмешательство, коронарное шунтирование, каротидная эндартерэктомия, компьютерная электроэнцефалография.
