Safety and immunogenicity of Biken acellular pertussis vaccine in combination with diphtheria and tetanus toxoid as a fifth dose at four to six years of age

Abstract: In comparison with a fourth dose of Biken DTaP administered at 18 to 24 months of age in the same population, the rate of local reactions increased after the fifth dose, whereas systemic reactions remained similarly low or decreased.

“…44,45 Local reactions to booster doses of tetanus toxoid were linked to high preimmunization antitoxin concentrations. 46,47 Local reactions to booster doses of DTaP vaccines were associated with elevated preimmunization values of diphtheria, tetanus or pertussis antitoxins in one study, 18 as in the present report, but not in others, 16,17 although few reports to date have had sufficient power to assess uniform vaccine sequences. Cell-mediated immunity (CMI) to toxoids develops with sequential dosing, 48,49 and CMI to diphtheria and pertussis toxins was associated with large reactions to DTaP boosters in one report.…”

“…An acellular bicomponent vaccine containing PT and filamentous hemagglutinin (FHA), was shown to be safe and effective in Japan (29). Purification of other B. pertussis components led to several acellular licensed vaccines composed of one to five components (30,31). The combination of purified proteins, including diphtheria, tetanus, and pertussis proteins and Haemophilus influenzae type b conjugate in one vaccine induced Arthus reactions in recipients of more than three injections of the combination vaccines (32,33).…”

“…88 A significant body of data exists showing a direct correlation between elevated pre-vaccination IgG levels and local reaction. These data are derived from studies with tetanus toxoid, 28,29,[89][90][91][92][93] diphtheria toxoid, 94-100 whole cell pertussis, 101 diphtheria/tetanus/acellular pertussis (DTaP) 38,102 and pneumococcal polysaccharide vaccine. 57,81,[103][104][105][106][107][108][109][110] Inflammatory response follows the formation of immune complexes between the circulating antibodies and the injected antigen.…”
