Safety and Efficacy of Therapies for Skin Symptoms of Psoriasis in Patients with Psoriatic Arthritis: A Systematic Review

Abstract: Numerous guidelines and recommendations exist for the treatment of psoriasis in various populations. One important population is patients with psoriatic arthritis (PsA) who have symptoms of both joint and skin disease. In patients with both facets of psoriatic disease, skin and joints must be treated separately, but also simultaneously. As several systemic therapies are approved for either one or both, the concept of treating both facets with the same drug is feasible. This review summarizes evidence from stud… Show more

“…Subsequently, we posted the principles online for further comment and to obtain agreement among GRAPPA membership. GRAPPA members updated the reviews that were previously published , reviewing subsequent literature within 6 subgroups addressing peripheral arthritis, axial disease, enthesitis, dactylitis, psoriasis, and nail disease in the setting of PsA. A central literature search was performed in collaboration with the University of Leeds Library in 2013 as detailed previously , with relevant articles provided to the groups.…”

“…In addition, a representative from each group focused on treatment safety. Each group then conducted a systematic literature review of the PsA treatment literature and excerpted and published the evidence base for treatment effect for each of the domains . We applied the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) approach to formulate these recommendations , which included determination of the strength of recommendation of each therapy (strong, conditional) based on the quality and breadth of evidence that the treatment can achieve desirable effects (e.g., reduction of morbidity and mortality, improvement in quality of life, reduction in burden of treatment and resource utilization), contextualized to individual patient and societal considerations .…”

“…The role of anakinra remains uncertain. Psoriasis (for details, see Boehncke, et al 10 ). To comprehensively treat patients with both PsA and psoriasis, we must control both disease domains.…”

“…It is also important to note PSAI and many other instruments are not developed on a metric scale, which introduce systematic bias to the studies that included patients with mild skin disease [Leung et al 2014b]. A systematic review by GRAPPA, which included 25 studies, revealed both DMARDs (MTX, LEF, cyclosporine A) and biologics (mainly anti-TNFs) have some efficacy as therapy for skin PsO [Boehncke et al 2014]. Looking forward, it will be important to develop appropriate outcome measures on metric scale to be used in clinical trials.…”
