Aim: To compare the control of blood pressure after oral alpha methyldopa versus oral labetalol for management of gestational hypertension. Study design: prospective, comparative, observational study. Place & duration: The study was conducted at Department of Obstetrics & Gynecology, Central Park Teaching hospital, Lahore for six months i.e. 1st May 2018 to 31st October 2018. Methodology: Total 150 females were included in the study from OPD after fulfilling the inclusion and exclusion criteria. Then females were randomly divided in two groups by using lottery method. In group A, females were given labetalol orally 100mg 2 times a day for 7 days. In group B, females were given 250mg methyldopa 3 times a day for 7 days. The females were followed-up in OPD after 8 days. After 8 days, females were evaluated for systolic blood pressure (SBP) & diastolic blood pressure (DBP). Results: In this study we compared labetalol with methyldopa for management of gestational hypertension. SBP (Labetalol: 123.41±7.42 vs. Methyldopa: 126.62±7.33, p-value=0.009) as well as DBP (Labetalol: 77.18±4.39 vs. Methyldopa: 79.64±5.9, p-value=0.005) were better controlled in patients received labetalol than alpha methyldopa. Conclusion: Labetalol is better than methyldopa in lowering blood pressure (systolic & diastolic) in women for management of gestational hypertension. Hypertensive disorders in pregnancy (HDPs) are no longer seen as “transitory diseases cured by delivery.” It accounts for up to 50% of maternal deaths. Information concerning HDPs is less in developing countries like Ghana. This study was conducted to find out the prevalence, awareness, risk factors, control, and the birth outcomes of HDPs. This was a retrospective cohort study conducted among pregnant women seeking care in selected health facilities in the Ashanti Region. Data on demographics, HDPs, and its associated birth outcomes were collected. Logistic regression models were used to examine the association of the independent variables with HDPs. The burden of HDPs was 37.2% among the 500 mothers enrolled with chronic hypertension superimposed with preeclampsia accounting for 17.6%, chronic hypertension, 10.2%, and preeclampsia 6.8% whilst gestational hypertension was 2.6%. It was observed that 44% (220) of the mothers had excellent knowledge on HDPs. Oral nifedipine and methyldopa were frequently used for HDP management, and it resulted in a significant reduction in HDP burden from 37.2% to 26.6%. Factors that influenced the increased risk of HDPs were grand multigravida ( AOR = 4.53 ; CI = 1.42 –14.42), family history of hypertension ( AOR = 3.61 ; CI = 1.89 –6.90), and the consumption of herbal preparations ( AOR = 2.92 ; CI = 1.15 –7.41) and alcohol ( AOR = 4.10 ; CI = 1.34 -12.62) during pregnancy. HDPs increased the risk of preterm delivery ( AOR = 2.66 ; CI = 1.29 –5.89), stillbirth ( AOR = 12.47 ; CI = 2.72 –57.24), and undergoing caesarean section ( AOR = 1.70 ; CI = 1.10 –2.61) amongst mothers during delivery. The burden of HDPs is high amongst pregnant mothers seeking care in selected facilities. There is the need for intensified campaign on HDPs in the Ashanti Region of Ghana.

Objective of this study was to compare efficacy and safety of Labetalol and Methyldopa in controlling blood pressure in patients with PIH and pre-eclampsia. Methods: A comparative, prospective observational, single centre study conducted from November 2015 to November 2017 in women with PIH at Indira Gandhi Government Medical College, Nagpur. Group A included 100 patients treated with Labetalol while Group B included 100 patients who were given Methyldopa. Response in lowering of BP was assessed over a period of 7 days. Results: Labetalol treated group of patients showed significant fall from 143.50±7.30mmHg/101.30±3.93 (sytolic/diastolic) on 1 st day to 126.10±5.49 mmHg/87.40±5.62 mmHg (sytolic/diastolic) on day 7, while systolic/diastolic BP in methyldopa group on 1 st day was 145.20±7.17 mmHg/101.60±4.20 mmHg which was reduced to 129.20±4.86 mmHg/90.50±3.30 mmHg on day 7. Author found that MAP in Labetalol group reduced from 115.226±4.17 mmHg to 100.17±4.43 mmHg on day 7 while in Methyldopa group had MAP on admission 115.99±4.38 mmHg and on day 7 it reduced to 103.27±2.99mmHg which is highly significant. Conclusions: Labetalol controls systolic and diastolic blood pressure more rapidly and effectively than Methyldopa. Safety profile and adverse effects of Labetalol and Methyldopa are similar to each other.
