Safety and Complications of Interventional Endoscopic Ultrasound

Abstract: Endoscopic ultrasound (EUS) has become an essential tool for the diagnostic and therapeutic intervention of gastrointestinal diseases. Beyond the drainage of fluid collections, it enables decompression of inaccessible bile and pancreatic ducts, the gallbladder, and the creation of anastomosis within the gastrointestinal tract using fully lumen-apposing stents. This review explored the safety and efficacy of these novel procedures and discussed the training pathway that is necessary to perform them efficiently … Show more

“…Though EUS is an invasive procedure, it has a low complication rate as a diagnostic tool. The safety of EUS alone as a diagnostic procedure is not as well studied as EUS in conjunction with interventional procedures, such as fine-needle aspiration [26]. However, a study involving the review of 43,852 patients found only 16 (0.03%) had perforation in the cervical esophagus [27].…”

“…There was no difference in the incidence of perforation or bleeding among the two groups on any statistical test applied and these specific complications have been studied in both group of patients in the past as well by Nabi et al, and Saumoy et al 13,14 . Similar results were obtained in case series published by Malik et al Which showed that laparoscopic procedure was a safe open surgical method for peri pancreatic collections and average hospital stay was 4.1 ± 2.3 days 19 .…”

“…Previous studies have clearly demonstrated a better safety profile of endoscopic procedures for peri-pancreatic fluid collections but many complications have been seen to arise especially due to poor interventional or endoscopic skills in the management of biliary pathologies or due to unavailability of resources, facilities and skilled staff. Perforation, bleeding and secondary infections are some of the commonly occurring complications in the patients undergoing endoscopic procedure for the drainage of peri-pancreatic fluid collections 13,14 .…”

“…In comparison with diagnostic EUS, interventional EUS has a higher risk of complications due to operator-based experience and procedure difficulty. Nevertheless, interventional EUS has an overall success rate of more than 90% [ 43 ].…”
