This study aimed to evaluate the feasibility of a 3-dimensional (3D) planned surgical guide technique designed for impacted supernumerary teeth (STs) extraction using 3D printed simulation models. In total, 17 participants from two university hospitals were recruited. Each participant performed surgery with both the conventional and surgical guide techniques. The following parameters were evaluated: (1) The time required for ST extraction, (2) the area of the window opening, and (3) the volume of the material removed. Time required for ST extraction was 213.65 ± 167.45 sec and 114.76 ± 42.87 sec in the conventional and surgical guide techniques, respectively, with significant differences (p = 0.028). The required area of the window opening was 48.10 ± 9.44 mm2 and 45.90 ± 8.17 mm2, respectively, in the conventional and surgical guide techniques, with no significant differences (p > 0.05). The required volume of the material removed was 121.65 ± 43.69 mm3 and 99.12 ± 17.88 mm3 in the conventional and surgical guide techniques, respectively, with significant differences (p = 0.031). The 3D planned surgical guide technique was effective for impacted ST extraction. Within the limits of this study, the surgical guide technique allows us to accomplish minimally invasive surgery within a shorter duration.

Mesiodens are supernumerary teeth located between the central incisors, close to the midline; Generally, they have a conical shape, can be impacted, and cause aesthetic-functional imbalance of the stomatognathic system. It is essential to emphasize that early diagnosis, through clinical and complementary exams, provides an optimized therapeutic strategy. The aims of the present study were to report a clinical case on strategies used to delimit the surgical opportunity as well as operative procedures performed in the presence of impacted mesiodens in a child patient. An 11-year-old female patient presented to the dental clinic for evaluation of a mesiodens-type supernumerary tooth. During the clinical examination, no areas of swelling were evident that would indicate the presence of the supernumerary. During the complementary examination, carried out using a panoramic x-ray in semi-open occlusion and ConeBeam® computed tomography, a supernumerary tooth of the mesiodens type can be seen impacted between the roots of the permanent maxillary central incisors. The mesiodens had an intimate relationship with the superior cortex of the nasopalatine canal, promoting luminal stenosis and an intimate relationship with the floor of the nasal fossa. Exodontia was chosen as a therapeutic strategy. Management strategies were reinforced until the surgical procedure session. The patient and legal guardian received post-operative guidance, were monitored for 15 days after the procedure, and were informed of the importance of monitoring until complete rhizogenesis of the permanent maxillary central incisors. In conclusion, it is highlighted that the surgical opportunity needs to be well-defined.

Rationale: Removal of impacted supernumerary teeth requires precision and accuracy to prevent iatrogenic injury to important anatomical structures during dental surgery and to improve postoperative healing. Patient concerns: A 12-year-old girl visited our department for the assessment and management of her deviated front teeth. Diagnosis: Impacted supernumerary tooth extraction in the maxillary anterior region. Interventions: The digital guide plate was fabricated after the integration of cone beam computed tomography data with that obtained from scanning the patient's dental model. Impacted supernumerary tooth extraction was performed. Outcomes: The use of the digital guide plate and planting instruments made the removal of the impacted supernumerary tooth less invasive, faster, and more accurate, whereas the wound was smaller, and the patient experience more comfortable. Lessons: Combining the digital guide plate with planting instruments offers a useful aid for the removal of impacted supernumerary teeth among the maxillary anterior region and is, thus, worth promoting.
