Safe balloon inflation parameters for resuscitative endovascular balloon occlusion of the aorta

Abstract: BACKGROUND: Noncompressible hemorrhage is a leading cause of preventable death in civilian and military trauma populations. Resuscitative endovascular balloon occlusion of the aorta (REBOA) is a promising method for controlling noncompressible hemorrhage, but safe balloon inflation parameters are not well defined. Our goal was to determine the balloon inflation parameters associated with benchtop flow occlusion and aortic/balloon rupture in ex vivo human aortas and test the hypothesis that optimal balloon infl… Show more

“…One ex-vivo model demonstrated balloon inflation volumes ranging from 4 to 22 mL for complete zone 1 aortic occlusion. 9 The appropriate balloon volume to achieve complete REBOA matters because the physiology of aortic blood flow is highly dynamic and influenced by multiple intrinsic physiologic and extrinsic factors. 10 In patients suffering from hypovolemic shock, the process of achieving and maintaining complete aortic occlusion is made more complex by changes in circulating endogenous catecholamines, vascular tone, cardiac output, and central aortic pressure.…”
