Saccadic vector optokinetic perimetry in children with neurodisability or isolated visual pathway lesions: observational cohort study

Abstract: While SVOP is highly accessible to children, many cannot complete a full 40-point test. Agreement with current standard tests is moderate to poor. Abnormal saccades cause an apparent non-specific visual field defect. In children with glaucoma or nystagmus SVOP calibration often fails.

“…These data indicate that testability was limited in the full test protocol of 40 points, but improved with reduced plots such as the 14‐point protocol used in this study. This aligns with the only other publication of SVOP in children with neuro‐disabilities and visual field defects …”

“…SVOP relies on fixation stability to determine whether or not stimuli have been ‘seen’, yet nystagmus – a specific issue for children with visual impairment – results in fixation instability for the majority of patients . These results replicate the findings of Tailor et al on the incompatibility of SVOP with nystagmus. Development of eye‐tracking devices that can detect and correct for nystagmus would be hugely beneficial.…”

“…This compares favorably with a study performed by Tailor et al, 32 in which only 12.5% of child patients completed a 40-point SVOP test. The children in the Tailor et al 32 study had quite severe neurodisability.…”

“…Among the 118 papers, 76 did not mention that the GMFCS was developed specifically for use with children with CP or discuss its measurement properties . Twenty noted that the GMFCS was created for children with CP but did not justify its application to their condition/clinical description .…”
