Since the eruption of the worldwide SARS-CoV-2 pandemic in late 2019/early 2020, multiple elective surgical interventions were postponed. Through pandemic measures, elective operation capacities were reduced in favour of intensive care treatment for critically ill SARS-CoV-2 patients. Although intermittent low-incidence infection rates allowed an increase in elective surgery, surgeons have to include long-term pulmonary and extrapulmonary complications of SARS-CoV-2 infections (especially “Long Covid”) in their perioperative management considerations and risk assessment procedures. This review summarizes recent consensus statements and recommendations regarding the timepoint for surgical intervention after SARS-CoV-2 infection released by respective German societies and professional representatives including DGC/BDC (Germany Society of Surgery/Professional Association of German Surgeons e.V.) and DGAI/BDA (Germany Society of Anesthesiology and Intensive Care Medicine/Professional Association of German Anesthesiologists e.V.) within the scope of the recent literature. The current literature reveals that patients with pre- and perioperative SARS-CoV-2 infection have a dramatically deteriorated postoperative outcome. Thereby, perioperative mortality is mainly caused by pulmonary and thromboembolic complications. Notably, perioperative mortality decreases to normal values over time depending on the duration of SARS-CoV-2 infection.

Zusammenfassung Hintergrund und Ziel Durch ihre Tätigkeit sind Menschen aus medizinisch-pflegerischen Berufen einem erhöhten Risiko für eine SARS-CoV-2-Infektion ausgesetzt und dadurch öfter von Krankheitsfolgen betroffen. In bisherigen Studien wurde als häufigste Krankheitsfolge die postvirale Fatigue (Erschöpfungssyndrom nach viraler Infektion) identifiziert. Das Ziel der Studie war die Untersuchung von Risikofaktoren für anhaltende Fatiguesymptome infolge einer COVID-19-Infektion und deren Auswirkungen bei Beschäftigten im Gesundheitswesen. Methoden Im Frühjahr 2021 wurden 4315 Versicherte der Berufsgenossenschaft für Gesundheitsdienst und Wohlfahrtspflege (BGW) für eine schriftliche Befragung zu ihrer COVID-19-Erkrankung im Jahr 2020 und den Krankheitsfolgen angeschrieben. Dabei wurden Symptome der Akutinfektion, Krankheitsfolgen, mögliche Risikofaktoren sowie der körperliche und psychische Gesundheitszustand nach der SARS-CoV-2-Infektion erhoben. Als Fatiguescreening wurde die Skala „Allgemeine Erschöpfung“ des Multidimensional Fatigue Inventory (MFI) eingesetzt. Zur Datenanalyse wurden Regressionsanalysen und multivariate Varianzanalysen berechnet. Ergebnisse 10,7 % der Befragten wiesen schwere Fatiguewerte auf. Als Risikofaktoren für eine klinische Fatiguesymptomatik konnten u. a. Vorerkrankungen der Psyche und Atemwege sowie die Schwere der Akutinfektion identifiziert werden. Weiterhin war eine schwere Long‑/Post-COVID-Fatigue mit einer höheren psychischen Belastung, einer niedrigeren gesundheitsbezogenen Lebensqualität sowie mit einer häufigeren Arbeitsunfähigkeit assoziiert. Diskussion Von schwerer Long‑/Post-COVID-Fatigue geht ein hoher Leidensdruck aus, der spezifische Rehabilitationsansätze erfordert und Sozialversicherungsträger sowie Unfallversicherer vor die Herausforderung stellt, geeignete Rehabilitationskonzepte zu entwickeln.

The frequency and extent of persistent sequelae in children and adolescents after infection with SARS-CoV-2 still needs to be comprehensively determined. In this cross-sectional clinical trial, we used non-invasive, label-free morphologic and free-breathing phase-resolved functional low-field magnetic resonance imaging (LF-MRI) to identify pulmonary changes in children and adolescents from 5 to <18 years after previously PCR-confirmed SARS-CoV-2 infection. While morphological pathologies were less frequent in children, functional LF-MRI visualized widespread ventilation, perfusion and combined ventilation/perfusion defects compared to healthy controls. The loss of functional lung parenchyma was more pronounced in long Covid than recovered patients. While pulmonary dysfunction was persistent even month after primary infection, LF-MRI demonstrated high capability to visualize and detect these changes in children and adolescents.
