Ruptured inferior vena cava aneurysm in the setting of mural vascular malformation: A case report

Abstract: Aneurysm of the inferior vena cava is a rare anomaly with a very few reported cases worldwide. We report the case of a 26‐years‐old man with acute severe abdominal pain and hypovolemic shock following an episode of syncope. Ultrasonography showed a fusiform aneurysmal dilation of the infra‐hepatic inferior vena cava (IVC), with a large saccular portion at its posterolateral wall and mural thrombosis. Abdominal computed tomography scan revealed extension to the right renal vein and adhesion to the right kidney.… Show more

“…After reviewing the literature, we found just one case report of a ruptured inferior vena cava aneurysm, and none of ruptured superior vena cava or azygos vein aneurysm. 8 After resecting her aneurysm and reconstructing the superior vena cava, postoperative imaging showed a good surgical outcome without relevant stenosis and a completely eliminated aneurysmal sack, followed by a smooth recovery until discharge.…”

“…[19,20] A retrievable IVC filter plays a crucial role, as it prevents PE and is recommended for the therapy of the iliofemoral DVT via Reya system in patients who have scheduled for interventional treatment. [21] Nonetheless, the patients who have contraindication to replace a IVC filter such as aneurysm of IVC, which reported rarely in the literature, [22][23][24][25] cannot be treated via a combined PMT and CDT therapy. In our series, a huge aneurysm of IVC was observed and the combined therapy failed due to an abortive IVC filter insertion (Figure 3).…”

“…They may be discovered incidentally in asymptomatic patients, as it was in our case, or may present with complications, such as leg swelling, pulmonary embolism and retroperitoneal mass related effects, like abdominal/low back pain, hydronephrosis and bowel obstruction [ 3 5 ]. Because the symptoms are non-specific, imaging tests like ultrasonography, contrast-enhanced CT, magnetic resonance imaging and cavography play a vital role in the diagnosis of IVC aneurysms [ 6 ]. However, some IVC aneurysms could be confused with retroperitoneal tumors, such as renal carcinoma, sarcomas, enlarged lymph nodes, neurogenic tumors and primary IVC tumors [ 7 9 ], especially when the lumen of the aneurysm is completely thrombosed [ 8 ].…”
