RSI in pediatric anesthesia – is it used by nonpediatric anesthetists? A survey from south‐west England

Abstract: Classical RSI is used for children by most anesthetists in south-west England. RSI is modified for infants especially by more recently trained consultants. Suxamethonium is used less by consultant anesthetists. Whilst RSI is performed for appendicectomy there is a large variation in techniques for anesthetizing children for MUA and scrotal exploration which is independent of the grade of anesthetist.

“…‘Classic’ RSII is often performed in children, especially by nonpediatric anesthesiologists, despite the fact that their priorities are different. A wide range of ‘pediatric’ RSII techniques are available indicating a lack of consistency and nonapplicability of the ‘classic’ technique in children . A depolarizing muscle relaxant is advocated as it rapidly leads to complete relaxation and adequate intubation conditions.…”

“…Por outro lado, em estudo sobre a aplicação da manobra de Sellick em crianças, apenas 59% dos anestesiologistas entrevistados afirmaram empregar a técnica. Quando a idade considerada era a escolar, a freqüência observada foi de 96% 30 . Quando aplicada de forma incorreta, a manobra de Sellick pode dificultar a intubação e a ventilação 31 .…”

Considering only school age children, 96% reported the use of the technique 30 . When the Sellick maneuver is not applied properly, it can hinder intubation and ventilation 31 .…”

“…[4] The surveyed doctors were more likely to utilize CP in pyloromyotomies than for scrotal explorations, which intuitively seem reasonable. Pediatric anesthesia practitioners in USA and UK have variable practice in doing RSII, even in non-fasted patients.…”
