Routine minimalist transcatheter aortic valve implantation with local anesthesia only

Abstract: Aims Conscious sedation instead of general anesthesia has been increasingly adopted in many centers for transfemoral transcatheter aortic valve replacement (TAVR). Improvement of materials and operators’ experience and reduction of periprocedural complications allowed procedural simplification and adoption of a minimalist approach. With this study, we sought to assess the feasibility and safety of transfemoral TAVR routinely performed under local anesthesia without on-site anesthesiology support. … Show more

“…7 The use of transcatheter aortic valve implantation (TAVI) for AS has significantly increased in recent years 8 and a minimalist approach with local anesthesia can be used in most cases. 9 Advances in technologies and new techniques are developing to prevent possible complications. 10 Although, as mentioned above, diagnosis of AS is mostly based on echocardiographic assessment, computed tomography (CT) is essential for proper TAVI planning.…”

“…So far, great efforts have been made in order to identify possible clinical and procedural predictors of uncomplicated vs. complicated clinical courses, in order to set up simple and streamlined pathways for the post-TAVI optimal management. [2][3][4][5] However, mid and long-term outcomes data are still lacking (Fig. 1).…”

“…The LA-only approach is also feasible thanks to improvements in operators’ experience [ 5 ], and was previously found to be associated with a reduced length of stay [ 6 ]. We aimed to evaluate differences between LA-only versus CS approaches on short- and long-term outcomes among patients undergoing TAVI.…”
