Rotational Acetabular Osteotomy for Osteonecrosis with Collapse of the Femoral Head in Young Patients

Abstract: In the present study of young patients with extensive osteonecrosis and collapse of the femoral head, rotational acetabular osteotomy was associated with symptomatic relief and absence of substantial collapse of the femoral head at the time of early to intermediate-term follow-up.

“…Recently, rotational acetabular osteotomy for ONFH was shown to provide symptomatic relief and to prevent substantial collapse of the femoral head at the time of early to intermediate follow-up [14]. Although improved acetabular coverage is a common result of both rotational acetabular osteotomy and acetabuloplasty, their utility in treating ONFH can differ.…”

“…Although improved acetabular coverage is a common result of both rotational acetabular osteotomy and acetabuloplasty, their utility in treating ONFH can differ. Rotational acetabular osteotomy is indicated for ONFH with some degree of an intact lateral cortex of the femoral head [14]. On the other hand, ARO combined with acetabuloplasty is indicated for ONFH cases with one third or more intact posterior area of the femoral head.…”

“…An RAO for AD usually has a satisfactory outcome [25,26]. However, since some cases show poor outcomes, studies have sought to identify the causes of these poor outcomes.…”

“…In Caucasians, oestrogen receptor (ER) and vitamin D receptor (VDR) gene polymorphism may be associated with AD, although no statistical significance has been observed [19]. An rotational acetabular osteotomy (RAO) is frequently performed to treat patients with symptomatic AD who show less than 20°of centre-edge (CE) angle to forestall the expected progress of degenerative change of the hip joint [25][26][27]. Although most of the patients that undergo RAOs show satisfactory results, some have poor results, as described below [27].…”
