Ross procedure in congenital patients:Results and left ventricular function1

Abstract: The Ross operation is a safe procedure and allows us to suppress completely the abnormal loading conditions of the left ventricle, resulting in a complete recovery of left ventricular function in most patients.

“…Rubay and colleagues [14] also observed distal anastomotic stenoses with homografts in the Ross series, and the same phenomenon was also observed after repair of truncus arteriosus with Gore-Tex# non-valved conduits [15]. Contegra# bovine jugular vein conduits were reported to trigger neo-intimal proliferation at the distal anastomosis, leading to increasing RV to PA gradient with progressive conduit dilatation and valve incompetence [16][17][18][19].…”

“…12 The significant change in the loading conditions of the left ventricle may result in myocardial remodeling and improved ventricular performance. 13,14 This remodeling may have an effect on exercise performance in the early period of follow-up.…”

“…However, most events refer to autograft or homograft replacement. Because the Ross procedure virtually abolishes the systolic pressure gradient across the aortic valve, it is not surprising that several studies have shown good return of left ventricular function after the Ross procedure [203,208,252], better than after valvotomy or valve replacement [208]. After the Ross procedure in children there is evidence for continued growth of the aortic annulus [203,215,220,246,253,254].…”
