Rationally selected Comprehensive System (CS; Exner, , 1993Exner, , 2003 Rorschach variables and suitability for psychodynamic psychotherapy were investigated in four studies with different designs. The participants comprised three clinical samples (n = 52, 25, and 69) of patients applying for and/or selected for dynamic psychotherapy. With few exceptions the CS variables were the same in the studies, and in all the studies the Rorschach was independent of clinical ratings/assessments and/or selection of patients for treatment. The first study dealt with the relation between 17 rationally selected CS variables and suitability for psychotherapy as represented by ratings of Dynamic Capacity and Ego Strength and by selection of therapy applicants for psychotherapy. The second and third studies concerned CS score differences between groups of patients differing in suitability for dynamic psychotherapy. In the second study, differences for CS variables were predicted for three groups of dynamic psychotherapy patients with mixed diagnoses. In the third study, CS score differences between two groups, differing in suitability but similar in comprising personality disordered patients, were predicted and tested. In the fourth study, correlations were predicted between 18 pre-therapy CS variables and patient post-therapy satisfaction with therapy and also with therapist post-therapy ratings of their patients' suitability for dynamic therapy.For the four studies taken together, positive results were obtained for EA, FC, the D Score, Blends, Zf, F%, MQo, and AG. The results for m, SumT, WSUM6, X-%, S, and FrrF were negative, whereas the results for YFY, FD, COP, and PER were too weak to permit interpretation.The main limitation of the research in this thesis is the lack of psychometric data concerning the clinical ratings whereas the strength is the ecological validity. The results are positive for some of the CS variables selected, mainly concerning control and cognition but also touching upon emotional integration and interpersonal forcefulness. The usefulness of the CS in psychotherapy assessment may be enhanced if only those CS variables that are found valid for the purpose are used and general personality descriptions are avoided.

The present study tested the hypothesis that Rorschach measures of psychological instability and perceptual sensitivity (i.e., es, es-EA, Lambda, Blends, Zf, & Zsum) would be sensitive to therapeutic change after inpatient treatment of children. Twelve male children were administered the Rorschach Ink Blot test in accordance with Exner's (1986) procedures and comprehensive system, pretreatment and posttreatment. The 12 children received inpatient treatment an average of 12 months, and all children showed improved functioning after treatment. Pre-to posttreatment Rorschach performance changes provided a partial confirmation of the hypothesis. Specifically, significant changes in es and Lambda were found posttreatment. As such, these Rorschach variables appear to be sensitive to therapeutic change in children.A child's performance on the Rorschach Ink Blot test has been shown to be predictive of later psychological adjustment (Tuber, 1983) and to be of prognostic value for the prediction of therapeutic outcome of inpatient treatment (LaBarbera & Cornsweet, 1985). Furthermore, Beck and Worland (1983) have shown that scores on a developmental level (DL) index, derived from Werner's theory of perceptual/analytic development, differentiated children of psychotic parents from children of nonpsychotic parents; children of psychotic parents showed less mature visual perceptual/analytic abilities. However, DL scores were not predictive of the need for later psychiatric treatment.Nevertheless, LaBarbera and Cornsweet (1985) did demonstrate that Rorschach indices of psychological instability and perceptual sensitivity were predictive of therapeutic outcome of inpatient treatment. In this study, children who had completed inpatient treatment were classified as "improvers" or "decliners" and were compared on indices of psychological instability and perceptual sensitivity. Significant group differences were found where "improvers" and "decliners" were compared on six Rorschach variables: es, es-EA, Lambda, Blends, Zf, and Zsum. With the exception of Lambda, those children classified as improvers showed higher mean scores on all of the above variables. The results of LaBarbera and Cornsweet suggest that those children who were the least stable, but were perceptually sensitive showed the greater therapeutic benefit from inpatient treatment.The aforementioned indices were derived from Exner's (1986) comprehensive system and may be interpreted as follows:es. Empirical support for interrater reliability of Rorschach Comprehensive System coding

Although a great deal of data has been published in the past 20 years supporting the interrater reliability of the Rorschach, recently commentators have raised anew concerns over the interrater reliability of this well-known and frequently used measure. An analysis of the literature reveals that these concerns are based on a selective review of the literature. The current study reports interrater reliability statistics for 70 interpretive Rorschach variables (n = 84), breaking down intraclass correlations statistics by base rate. Results confirm the strong empirical evidence of the interrater reliability of this measure when scored by a well-trained and diverse group of researchers and clinicians. Reliability is especially strong and consistent for the high base-rate variables from which clinicians often base their interpretations. These data further suggest that large samples are needed for stable reliability estimates of low base-rate variables and that estimates of the reliability of low base-rate variables are subject to error.
