ACTA FACULTATIS MEDICAE NAISSENSIS UDC: 616.314.6 SU M M A RYMorphological and anatomical characteristics of the tooth are of great importance for clinical dentistry. These aspects of the tooth can be better conceptualized from the perspective of external and internal morphology of dental crowns and roots.The aim of this study was to determine the average total length of the upper first and second premolars, the average length of crowns, the average length of roots and to determine the most common number of the roots and root canals of these teeth.We used 41 first and 48 second extracted upper premolars.The length of these teeth was measured by a digital micrometer. The number of roots was determined visually, and each tooth was longitudinally dissected across the vestibular-palatal plane for determining the number of root canals.The average total length of the upper first premolars was 22.5 mm, the average crown length was 8.8 mm, the mean root length was 13.7 mm. The upper first premolars usually have two roots in 53.7% and two root canals in 82.9%. The average total length of the upper second premolars was 22.5 mm. The average crown length was 8.4 mm and the mean root length was 14.1 mm. The upper second premolars usually have one root in 89.6% and one root canal in 79.2%.The determined average values of the length of these teeth and the length of their crowns and roots were similar to the values of the same population reported by other authors.

INTRODUCTION The objective of this study was to propose a root canal configuration description method and to investigate the root canal system morphology of the maxillary first molar by means of micro-computed tomographic imaging. METHODS The root canal configuration, foramina, and accessory canal frequency of 179 maxillary first molars were investigated by means of micro-computed tomographic imaging and 3-dimensional software imaging. The root canal configuration and main foramina number are described from coronal to apical with a 4-digit system. RESULTS The most frequent root canal configurations were 1-1-1/1 (45.8%), 2-2-2/2 (25.1%) and 2-2-1/1 (10.1%) in mesiobuccal roots and 1-1-1/1 in distobuccal (97.2%) and palatal (98.9%) roots. The first mesiobuccal (MB1) root canal had 1 accessory canal in 26.3% of the teeth, the distobuccal root canal had 12.3%, and the palatal root canal had 9.5%; in the second mesiobuccal root canal, there was rarely 1 accessory canal. There was 1 accessory canal in 26.3%, 12.3%, and 9.5% in the MB1, distobuccal, and palatal root canals, respectively. The MB1, distobuccal, and palatal root canals had 1 main foramen. The MB2 had 1 main foramen in 39.0% of the teeth and no main foramen in 61.0%. CONCLUSIONS The root canal configuration of maxillary first molars is quite diversified. Contrary to our expectations in this research, the mesiobuccal root has predominantly 1 root canal entrance and only 1 main foramen. Anatomic variations including connecting and accessory canals occur in any third of root.

The purpose of the study was to determine the effect of apical clearing and apical foramen widening in reducing apical ramifications and bacterial load in the apical third of root canals. The mesio-buccal roots of 21 maxillary first molar teeth were inoculated with Enterococcus faecalis suspension using a sterile pipette. Samples were incubated at 37°C for 72 hrs and divided into 3 groups: Group A, control group (n=5), no preparation; Group B (n=8) conventional preparation alone; and Group C (n=8), apical clearing and foramen widening in addition to conventional preparation. Bacterial counts were semi-quantitatively analyzed pre-and post-preparation. Samples were demineralized with 5% nitric acid after injection of India ink. Cross sections were obtained at every 0.5 mm from the apex to 3 mm of the root using a vibratome and viewed under a stereomicroscope at 64×magnification to locate any debris or apical ramifications. The Kruskal-Wallis and Mann-Whitney U tests were used for the statistical analysis. A statistically significant difference was observed (p value 0.006) in the number of ramifications among the 3 groups. Group C had a lower average number of ramifications (1) than Group B (2.5) or A (4). The debris score was analyzed at each level (0.5-3 mm). A statistically significant difference was observed at 0.5 mm and 1 mm between Group A and C (p=0.0041) and Group B and C (p=0.0050), whereas no difference was found between Group A and B (p>0.05). These results indicate that there was less debris and fewer apical ramifications in Group C. The microbiological study revealed a lower number of colony forming units (10 ). These results suggest that apical widening and clearing facilitates removal of apical ramifications and bacterial load within root canals.
