Role of staging laparoscopy with peritoneal lavage cytology in the treatment of locally advanced gastric cancer

Abstract: Background. More accurate preoperative staging is necessary to determine the treatment strategy for locally advanced gastric cancer. Laparoscopy has been suggested as an appropriate staging modality. The aim of this study was to clarify the role of staging laparoscopy in patients with locally advanced gastric cancer. Methods. One hundred patients with primary gastric adenocarcinoma underwent laparoscopy with peritoneal lavage cytology. The disease stages determined were compared with those obtained by conventi… Show more

“…Thus, staging laparoscopy has to be considered mandatory in the evaluation of GLP. It may have a role in confirming the diagnosis in doubtful cases, by demonstrating undiagnosed peritoneal seeding and by evaluating tumor resectability [13,14]. Explorative laparotomy should be avoided completely, if we consider the minimal life expectancy of these patients (median survival time 2.8 months).…”

“…Laparoscopy was reported to be an important diagnostic tool for the staging of advanced gastric cancer [20,21], and we too have performed it for patients with clinical T3 gastric cancer, by preoperative multidetector row CT since September 2002. Although staging laparoscopy facilitates a more accurate preoperative diagnosis of peritoneal dissemination, there is no standard treatment for peritoneal dissemination.…”

“…Current imaging studies rarely detect peritoneal deposits, although the detection of even minute amounts of cancer, such as those represented by positive cytology of peritoneal washes, usually renders the cancer incurable [9]. This knowledge prompted several investigators to perform staging laparoscopy prior to radical surgery [10]. However, it is not currently considered as a part of routine preoperative workup in Japan, where gastric cancer is one of the commonest types of cancer and is often treated at community hospitals in the hands of general surgeons.…”
