Role of scintigraphy with 99mTc-infliximab in predicting the response of intraarticular infliximab treatment in patients with refractory monoarthritis

Abstract: The results of the present investigation suggest that (99m)Tc-infliximab scintigraphy could be a useful tool to predict the clinical response to IA infliximab treatment in patients with refractory monoarthritis.

“…Quantitative analysis of the sacroiliac joints by bone scintigraphy with 99m Tc-MDP is limited by unsatisfactory visualization of the bone, which hampers subsequent discrimination of the sacrum and the sacroiliac joints from other regions of interest. Different groups have been using 99m Tc-infliximab to assess the degree of TNF-a expression in different rheumatological diseases [9,10].…”

“…Moreover, the clinical and ultrasonography assessmentconfirmed a total remission. After this case report, a larger population was evaluated including12 patients with active refractory mono-arthritis (11 knees and 1ankle), affected by RA (5 patients) and SpA (7 patients) (8). At baseline and after 12 weeks of treatment with intra-articular infliximab (100 mg for the knee and 50 mg for the ankle), a 99mTc-infliximabscintigraphy was performed.…”

“…Quantitative analysis of TNF in the inflamed sites was performed using the OSIRIS software. Target to background (T/B) ratios were calculated drawing circular region of interests (ROIs) on the affected joints (target) andrectangular ROIs approximately five cm below the knee or above the ankle [8]. The same ROIs wereused to analyse both pre-andpost-therapy images.…”

“…Anti TNF-a drugs can be labelled with 99m-technetium ( 99m Tc) and scintigraphy prior to therapy might predict whether this specific treatment will be successful. In rheumatoid arthritis, scintigraphy with 99m Tc-labelled infliximab demonstrated a significantly higher activity in the affected joints than in those that were not affected [77]. Studies evaluating the role of 99m Tc labelled anti TNF-a scintigraphy in sarcoidosis are lacking, although the success in other TNFmediated diseases suggests a promising technique.…”
