Background Biomarkers in head and neck squamous cell carcinoma (HNSCC) emerge rapidly in recent years, especially for new targeted therapies and immunotherapies. Methods Recent, relevant peer‐reviewed evidence were critically reviewed and summarized. Results This review article briefly introduces essential biomarker concepts, including purposes and classifications (predictive, prognostic, and diagnostic markers), and the phases of biomarker development. We summarize current biomarkers in order of clinical utility; p16 and human papillomavirus status remain the most important and validated biomarkers in HNSCC. The rationale for biomarker study design continues to evolve with technological advances, especially whole‐exome or whole‐genomic sequencing. Noninvasive body fluid and liquid biopsy biomarkers appear to hold strong potential for development as tools for early cancer detection, cancer diagnosis, monitoring of disease recurrence, and outcome prediction. In light of discrepancies among different technologies, standardized approaches are needed. Conclusion Biomarkers from cancer tissue or blood in HNSCC could direct new anticancer therapies.

Saliva is an emerging biofluid for personalized/precision medicine application. As it has recently become a major tool for biological research and biomarker discovery, it is of vital importance to develop and optimize the diagnostics standards to decipher the omics constituents in saliva in order to achieve better oral and systemic health. Although current variations in saliva sampling, study designs, and developmental methods of salivary biomarkers prevent direct comparisons of the data obtained from different studies, the lack of standardization and unification of the processing procedures undermines saliva's diagnostic potential. Thus, it motivated us to propose the idea of creating an International Scientific Foundation of Salivary Diagnosis (ISFSD) that could encourage collaborative efforts to establish international guidelines and standardized protocols for saliva collection and laboratorial processing as well as clinical diagnostics. The ISFSD can accelerate the validation of biomarkers in larger groups of patients, their regulatory approval, and implementation of saliva diagnostics into a real clinical practice. It will also result in identification of practical and achievable priorities, and will present policy initiatives critical to the implementation of oral health improvement programs. The establishment of ISFSD will be of great interest and influence in the fields of oral health, dentistry, and medicine.

Long noncoding RNAMEG3 inhibits oral squamous cell carcinoma progression via GATA3

Oral squamous cell carcinoma (OSCC) accounts for about 90% of oral cancers. Expression of the long noncoding RNA (lncRNA) maternally expressed 3 (MEG3) has previously been reported to be downregulated in OSCC, and its overexpression can inhibit proliferation, migration, and invasion and promote apoptosis of OSCC cells. However, the mechanism underlying MEG3 downregulation in OSCC has not been well characterized. Here we report that low expression of MEG3 is caused by H3K27me3 modification of the MEG3 gene locus, and this is associated with the poor prognosis of OSCC. Overexpression of MEG3 inhibited the proliferation and invasion of OSCC cells. We observed that MEG3 was modified by m6A and bound to YTHDC1. Enhancer-controlled genes positively regulated by MEG3 were functionally enriched for the 'negative regulation of Wnt signaling pathway' term, as determined using METASCAPE. GATA3 was predicted to be a transcription factor for these genes, and was demonstrated to bind to MEG3. Knockdown of GATA3 countered the effects on proliferation, invasion, and increased transcription of HIC1 and PRICKLE1 induced by MEG3 overexpression. In conclusion, our data suggest that MEG3 is downregulated in OSCC due to trimethylation of H3K27 at the MEG3 gene locus. The inhibitory effect of MEG3 on proliferation and invasion of OSCC cells was dependent on the binding of GATA3.Oral cancer is the sixth most common malignancy worldwide, with about 354 864 new cases and 177 384 deaths annually [1,2].
